Part-Time/Day Shift

The Community Health Worker (CHW) is a trusted community member who acts as a liaison between a community and health/social service systems. The CHW is knowledgeable about community resources and uses their lived experience to build trusting relationships with healthcare providers, community members, patients, and community partner organizations.

Required qualifications:

• Coursework/Training: Complete 80 hours training as Community Health Workers. Approved by the Oregon Health CHW Training curriculums.
• 1-3 years relevant community experience.

Posted are the minimum and the maximum wage rates on the wage range for this position. The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
