SUMMARY
This is a grant funded-temporary position with anticipated funding for two years. Continued employment beyond that time frame in this position is dependent upon grant funding availability.
The Community Health Worker (CHW) serves as a frontline liaison between MyMichigan Health and the Gratiot community. This role is responsible for conducting outreach and care coordination activities to connect patients with appropriate health care, community, and social service resources that address social determinants of health (SDOH) barriers and improve overall health outcomes. This person will be a key member of Gratiot County school-based clinics, ambulatory patient-centered health teams and community collaborations and will work closely with the Community Health and Population Health teams to support program goals and assist in meeting grant requirements.
Responsibilities:
- (25%) Collaborates with school personnel and community organizations to identify students in need, facilitate appropriate referrals, and connect students and families with school-based resources and social support services. Develops strong partnerships to promote positive student outcomes and continuity of care.

(25%) Manage Community Health work queue referrals for Gratiot County. Conduct patient outreach to address identified social determinants of health (SDOH) needs. Assist patients and families in addressing health disparities by connecting them with appropriate community resources to remove SDOH barriers.

(25%) Coordinates with the Community Health team to support community outreach events, health fairs, and other MyMichigan Health initiatives. Promotes health education, awareness, and access to services through outreach activities and community engagement. Serves as a liaison between schools, families, and community partners while representing and promoting CHW school-based services at off-site and community events.

(25%) Maintain current knowledge of SDOH best practices, CMS requirements, community resources, and referral processes. Utilize standardized Epic documentation workflows and templates to ensure accurate, compliant, and timely documentation of interventions, outcomes, and resource connections.
OTHER DUTIES AND RESPONSIBILITIES:
Support care team needs in school-based clinics/provider offices as appropriate to meet payor metrics and billing guidelines.
Establish/maintain positive relationships with community partners.
Collaborate with Community Health team if/when a need for a new community relationship(s) is identified.
Advocates for patients as needed. All other duties as assigned.
REQUIRED EDUCATION:
CHWCoreComp - Community Health Worker Core Comp
Required Equivalent Experience - within 6 months of hire or transfer
EXPERIENCE, TRAINING AND SKILLS:
- Experience in a health system, public health sector, or community benefit organization.

Must have strong written and oral communication skills, including preparation and report presentation.

Knowledge of personal computers, PC applications, MS Word, Excel, PowerPoint and Project.

Must be self-motivated, strong problem solver, and detail-oriented.

Effectively communicate with community members, partners and stakeholders.

Communicating health information using culturally appropriate terms and concepts.

Capability to navigate complex social/health issues.
PHYSICAL/MENTAL REQUIREMENTS AND TYPICAL WORKING CONDITIONS:
Exposure to stressful situations, including those involving public contact, as well as, trauma, grief and death.
Able to wear personal protective equipment that includes latex materials or appropriate substitute if required for your position.
Is able to move freely about facility with or without an assisted device and must be able to perform the functions of the job as outlined in the job description.
Overall vision and hearing is necessary with or without assisted device(s).
Frequently required to sit/stand/walk for long periods of time.
May require frequent postural changes such as stooping, kneeling or crouching.
Some exposure to blood borne pathogens and other potentially infectious material.
Must follow MyMichigan Health bloodborne pathogen and TB testing as required.
Ability to handle multiple tasks, get along with others, work independently, regular and predictable attendance and ability to stay awake.
Overall dexterity is required including handling, reaching, grasping, fingering and feeling. May require repetition of these movements on a regular to frequent basis.
Physical Demand Level: Sedentary. Must be able to occasionally (0-33% of the workday) lift or carry 0-10 lbs.
