JOB SUMMARY:
1. Supervises all aspects of consumer care and optimal functioning of TFH when on duty.
2. Carries out the supervision of Direct Support Staff in conjunction with the House Supervisors.
3. Keeps consumer record keeping system up to date on a timely basis.
4. Strives to provide, at all times, nursing services of optimal quality.
5. Functions as Charge Person when DON and Administrator are absent from the building.
SUPERVISED BY/REPORTS TO: Director of Nursing. Also responsible to Administrator/Executive Director.
EDUCATION/EMPLOYMENT QUALIFICATIONS:
1. Registered Nurse, graduate of an accredited school of nursing and possessing a current and valid license in the State of Maine.
2. Must not be a convicted felon or convicted of a crime involving abuse of a person or animal.
3. Knowledge and understanding of general nursing procedures and techniques.
4. Genuine interest in working with people with intellectual disabilities and/or developmental disabilities.
5. Training in/or willingness to learn skills of supervision.
PHYSICAL/SPECIAL DEMANDS:
1. Ability to lift 35 lbs. and assist with consumer lifting.
2. Ability to deal with stressful and emergency situations with calmness and tact.
3. Ability to maintain a good working relationship with all levels of personnel and function well in stressful and emergency situations with calmness and tact.
4. Ability to maintain confidentiality.
5. Ability to comply with State and Federal regulations and the facility’s Policies and Procedures.
WORK HOURS: Flexibility of hours is required in this position.
WORK ENVIRONMENT: Works primarily indoors in well-lighted and ventilated rooms. Possibility of cuts or minor burns from equipment. May be exposed to communicable diseases. Possibility of injury due to lifting, moving or interacting with consumers.

Specific Work Skills (These apply to one’s position):
Health Services Related
1. Is responsible for the basic health care of all consumers, i.e. vital signs, intake and output, as outlined in Nursing Manual.
2. Observes consumers to determine any new needs and any progress or regression.
3. Ensures all written records on changes in consumer's health status are completed, as outlined in the Nurse’s Manual.
4. Is aware of all new orders or change in orders for all consumers.
5. Assures physicians orders are transcribed and carried out correctly and effectively.
6. Makes rounds with physician and assists as indicated with procedures.
7. Communicates with and follows up with physicians regarding any changes in the consumer's status.
8. Completes nurse to nurse report on consumer status and house issues and MAR check at change of shift.
9. Completes controlled medication count with nurse or CNA/M at beginning and end of shift.
10. Administers medications and treatments as ordered by physician.
11. Encourages consumers to self-administer medications, as abilities allow and implements and documents programs as written.
12. Documents on the MAR's correctly.
13. Participates in the MAR to MAR checks at least twice yearly.
14. Is knowledgeable of written program plans (esp. self-administration and behavior plans), and implements, as needed.
15. Ensures that all accident/incident and reportable events reports are completed documented and reported as outlined. Follow up and document on injuries.
16. Is familiar with and understands the consumer's PCP.
17. Completes primary duties as outlined by DON. Is assigned a few consumers where the charge nurse assumes primary care for those consumers. On a quarterly basis completes the quarterly report and updates the care plan.
18. Ensures that reports related to primary consumers arrive in a timely manner, and that follow through is completed on any and all orders. Outstanding orders or labs need to be either taken care of directly or communicated to another nurse for follow through.
19. Maintains communication with day program with respect to any new information regarding consumer’s health.
20. Promotes independence and active treatment.
21. Coordinates consumer appointments and completes necessary paperwork and follow up.
Charge Related
1. Understands emergency protocol and initiate and follow through effectively. Supervises drills and completes reports.
2. Fills out tardy and absence forms when necessary and obtains coverage if a nurse, calls in or does not show. Nurses do the same for CNA staff. Provides nursing coverage until replacement nurse arrives. Ensures other departmental coverage is accurate when in charge.
3. Is aware of what is going on throughout the building.
On Duty Supervisory Related
1. Supervises direct support staff to include prioritizing work assignments, ensuring privacy, providing CNA care, etc.
2. Ensures appropriate reports and documentation are competed
3. Supervises CNA/M in regards to medication administration
4. Effectively oversees and QA’s nursing, program and administrative areas they are responsible for.
5. Communicates issues with co-supervisors
6. Consults written polices, HR, union contract, as needed
7. Knows and complies with State and Federal regulations.
8. Has effective leadership skills and participates in working toward the Agency mission statement.
9. Facilitates in-services and meetings as needed.
10. Ensures that Departmental resources and supplies are available at all times.
11. Differentiates and appropriately responds to issues as they come up; addressing immediate issues and deferring non-immediate issues to the appropriate supervisor.
12. Flexible and picks up shifts when asked.
Supervision of assigned positions (This section does not apply to Per Diem Nurses)
1. Works cooperatively with co-supervisor and all supervisory staff.
2. Participates in interviewing, hiring, and orientation of staff.
3. Consistently coordinates effective one to one and group supervision sessions with assigned staff.
4. Provides leadership, support, and direction to staff in accordance to TFH's supervisory philosophy.
5. Keeps DON/QMRP informed of successes and challenges.
6. Effectively follows the supervision and disciplinary process and takes needed steps to address and improve staff performance issues.
7. Completes performance appraisals on assigned staff.
8. Completes necessary documentation to support supervision to include CAP's and progressive discipline.
9. Participates in disciplinary process of employees in accordance with TFH policy and procedure and union contract obligations (i.e. recommended level of discipline, administration of discipline.)
10. Participates in Policy and Procedure and Instructional Manual development and review, and budget development.

You will provide and oversee nursing services for individuals with intellectual and developmental disabilities, supporting their health, independence, and overall well-being. In this role, you’ll collaborate closely with direct support professionals, supervisors, physicians, and interdisciplinary team members, offering guidance, clinical expertise, and calm leadership throughout each shift.
You’ll be hands-on with medication administration, health monitoring, care coordination, and documentation, while also serving as a resource and role model for staff. When leadership is off-site, you may step into an on-duty leadership role, helping manage the flow of the building, respond to emergencies, and ensure staffing and services remain strong.
