Tailored Care Management is a service which helps clients meet their health goals by coordinating their medical, social and behavioral health services. The Care Manager QP role is to improve positive client outcomes through linking clients to medication management, coordinating primary and behavioral health care needs, build trust between clients and their practitioners, connecting clients to community resources, enhance communication and the continuity of care

Job Duties Include:
- Performing general outreach, engagement, and follow-up with members
- Coordinating services/appointments
- Engaging in health promotion activities
- Providing and tracking referrals and providing information and assistance in obtaining and maintaining community-based resources and social support services
- Participating in case conferences
- Support the care manager in assessing and addressing unmet health-related resource needs
Basic Requirements:
- Ability to perform tasks on the computer including working with Microsoft 360, Excel, Word and other basic programs.
- Valid driver’s license, auto insurance coverage and reliable transportation
- Pass criminal background check and other regulatory checks
- Provide proof of your education
Education and Experience Required:
- A High School Diploma or equivalent
- Have at least one year of paid experience working directly with the TCM eligible population
- Meet One of the Following Requirements:
- Be a person with lived experience with an I/DD or a TBI with demonstrated knowledge of and direct personal experience navigating the NC Medicaid delivery system or,
- Be a person with lived experience with a behavioral health condition who is a Certified Peer Support Specialist or,
- A parent or guardian of an individual with an I/DD, TBI or a behavioral health condition and who has at least two years of direct experience providing care for and navigating the Medicaid delivery system on behalf of that individual (note that a parent/guardian cannot serve as an extender for his/her own family member).
Resume must include degree earned (major & minor), graduation month & year, relevant employment dates including month and year, and populations served.
