Job Description: LTSS Case Manager
Reports To: Senior Services Manager

JOB PURPOSE
The Long-Term Services Support Case Manager provides person-centered, long-term case management and service coordination for seniors, individuals with disabilities across the lifespan, and their caregivers. This role includes assessing needs, connecting consumers to services, and monitoring outcomes to support progress toward service plan goals. Travel within Northern Nevada counties is required.
KEY RESPONSIBILITIES
Client AssessmentCase Management
- Conduct initial and ongoing reassessments to identify consumer needs and appropriate long-term services.
- Develop, implement, and maintain individualized long-term case management service plans.
- Provide ongoing case management, including regular follow-up to monitor outcomes and adjust services.
AdvocacyPartnerships
- Advocate on behalf of consumers and their families with service providers and agencies.
- Serve as a trusted resource on long-term services and supports.
- Maintain collaborative relationships with public and private community partners.
DocumentationCompliance
- Document all client contacts, services, referrals, and outcomes in required databases.
- Ensure documentation is timely, accurate, and compliant with program standards and HIPAA requirements.
OutreachService Delivery
- Provide case management services in person, by phone, or email at approved locations.
- Conduct home visits and offsite outreach as needed to meet consumer needs.
- Participate in outreach, marketing, and public education activities, including community events.
Professional Responsibilities
- Maintain working knowledge of AHN programs, systems, and required databases.
- Communicate effectively with supervisors and internal departments.
- Attend staff meetings and supervision sessions.
- Adapt to changing priorities and perform other duties as assigned.
EDUCATIONEXPERIENCE REQUIRED
- Bilingual (Spanish/English) preferred.
- High School Diploma required; Bachelor's degree in social work, public health, psychology, or a related field preferred.
- Minimum 2 years of experience in case management, care coordination, or related field.
- Minimum 1 year of customer service experience.
- Minimum 1 year of experience working with seniors, people with disabilities, and/or diverse populations preferred.
- Proficiency in Microsoft Office and database systems.
- Medicare Assistance Program Level II Certification (must obtain within 60 days of hire).
- Valid driver's license and ability to operate an AHN company vehicle as needed.
- Flexibility to accommodate department-determined work schedules, which may change based on operational needs.
- Must adhere to HIPAA regulations.
WORK ENVIRONMENT
- The noise level in the work environment is usually moderate, and the employee usually works in a climate-controlled office environment.
- Ability to travel and drive for extended periods (up to four hours at a time).
- Be able to sit at a computer for 6 – 8 hours per day.
- Be able to stand between 3 – 6 hours per day.
- Be able to withstand hand/wrist deviation and repetition.
- Must be able to lift, carry up to 30 pounds with a fair consistency.
