What You’ll Do:
- Conduct psychosocial assessments and provide counseling to residents and families
- Support admissions, transitions, and adjustment to aging or illness
- Collaborate with the interdisciplinary team on care planning and documentation (including MDS)
- Assist with discharge planning and coordination of community resources
- Advocate for residents and provide crisis intervention as needed
- Maintain accurate documentation and support financial/insurance processes
- Serve as a liaison with families, agencies, and mental health services
What You’ll Need:
- You must have a Social Services Designee Certification or Social Worker degree
- At least 1 year of interdisciplinary team experience, with long-term care or hospice preferred
- Strong communication, critical thinking, and documentation skills
- Ability to work in a fast-paced, emotionally sensitive environment
- Proficiency with computer systems and maintaining confidentiality
- Ability to collaborate effectively with residents, families, and care teams
