GENERAL PURPOSE:
Under the direction of the HIV Program Manager, the Housing Opportunities for People with AIDS Housing Case Manager is accountable for providing intensive case management and crisis intervention services for those affected by HIV/AIDS. The Case Manager is responsible for ensuring compliance with contract services and ensures all clients are engaged in health care and additional services based on client need.
Main Responsibilities and Duties:
- Maintain confidentiality as per the Confidentiality Agreement.
- Conduct initial assessments to determine client eligibility and needs (via phone and in-person).
- Create and maintain service plans quarterly with well-developed goals using motivational interviewing and trauma-informed practices.
- Provide housing case management, client advocacy, and access to benefits & services to determine additional needs of the client.
- Refer eligible clients to the Registered Dietitian (RD) for medically tailored food services.
- Monitor client status to provide subsidized housing and case management to prevent homelessness.
- Maintain client documentation and ensure compliance with grant and program requirements.
- Maintain and update client documentation to meet program, policy, and grant requirements.
- Track and submit invoices for the program budget and ensure alignment with financial policies.
- Coordinate with Ryan White Part B non-medical Case Managers to avoid overlapping benefits for shared clients.
- Advocate for clients' needs within the community and service providers.
- Liaise with medical professionals and outside agencies as needed.
- Provide client education through pamphlets and resources.
- Participate in case conferences, CQI, and required training.
- Ensure peer reviews are conducted and collaborate with vendors for peer review processes.
- Attend trainings and meetings to maintain professional growth and ensure program compliance.
- Take and distribute meeting notes, track action items, and ensure follow-up.
- Evaluate and improve administrative processes for efficiency.
- Prepare for audits by maintaining accurate records and ensuring compliance.
- Other Duties as assigned by Supervisor
Qualities & Characteristics:
- Maintain professional relationships with staff, patients, and vendors.
- Uphold ethics, integrity, and enthusiasm for the organization's mission.
Knowledge, Skills & Abilities:
- High School Diploma (required), Medical Assistant (preferred).
- Proficient in Microsoft Office and able to type 40wpm.
- Strong communication, conflict resolution, and problem-solving skills.
Working Conditions & Physical Requirements:
- Ability to lift up to 40 lbs.
- Operate standard office equipment and handle physical tasks like bending, squatting, and reaching.
