AuthoraCare Collective is currently seeking a Hospice Nurse Practitioner. This is a full-time hybrid position; Monday - Friday 8:00am - 5:00pm supporting our Greensboro and Burlington service areas.
CPR certification from the American Heart Association or American Red Cross Basic Life Support (BLS) is required.
The Hospice Nurse Practitioner works in collaboration with the hospice physicians to perform hospice face-to-face encounters and provide with direct care for patients in our hospice inpatient units. The Nurse Practitioner is responsible for performing all duties in full compliance with all regulatory requirements including the North Carolina Nurse Practice Act and agency policies, procedures, and standards of care.

Education and Experience:
- Two years of experience required in an acute, home health, or long-term care or outpatient/office setting.
- Must be currently licensed as a Nurse Practitioner in North Carolina.
- Understand and be committed to the Hospice philosophy of care concepts. Ability to prepare for Certification as an Advanced Certified Hospice and Palliative Nurse.
- Excellent assessment and clinical skills and effective interpersonal and communications skills necessary.
Other: Valid state-issued driver's license required. Must carry automobile liability insurance at limits required by agency. Must have own transportation.
Key Responsibilities:
Provides hospice face-to-face encounters, including scheduling, performing the
encounter within agency deadlines, and documenting per federal requirements.
Provides direct care to inpatient unit patients in collaboration with the unit clinical team and hospice physician.
Participates in hospice interdisciplinary meetings if needed.
Documents in the patient medical record in accordance with federal and state regulatory requirements, standards of practice and agency policy
Collaborates with physicians, other health care providers, and interdisciplinary team members and makes timely reports of pertinent information to appropriate health care professionals. Communicates with courtesy and compassion to agency customers and employees and conduct self in a professional manner.
Consults and recommends referrals to other health care providers in the specialty area indicated by patient’s health care needs.
Maintains patient confidentiality at all times.
Participates in program integrity and performance improvement activities of the agency.
Performs other related duties including IPU care as directed by the Hospice Medical Director to promote the efficiency and effectiveness of the agency.
Enhance professional growth and self-development through participation in professional activities, educational programs, current literature, in-service programs, and workshops particularly as it applies to hospice patients.
Performs other duties as requested.
