Job Summary
The Relative Home Assessment Services / Relative Support Services (RHAS/RSS) Case Manager assists relatives and Non-Related Extended Family (NREF) Caregivers who have submitted an application for Resource Family Approval in the assessment process. The Resource Home Assessment Case Navigator’s main responsibilities are to provide orientation, training and conduct a home environment assessment, and when needed to write a corrective action plan and/or a documented alternative plan. Another fundamental aspect of this job includes the Relative Support Services (RSS) component. Case Managers are to advocate for caregivers and refer them to resources that will best assist them in various situations including finding food banks, mental health services, utility assistance, etc.
Essential Functions
- Visit families and conduct orientation, home inspection, and help with application process
- Provide 12 hours of training to upcoming resource families
- Provide 8 hours of recertification training within the first year of the approval date.
- Assess for items/furniture for families for home approval
- Scheduling family visits to assess caregiver’s needs e.g., Financial, clothing, food, etc.
- Creating a financial budget for the family
- Providing family with needed support
- Sensitive and responsive to the cultural differences among the agency’s client population
- Works to promote a trauma-informed work environment.
- Actively participate in quality improvement projects
The RHAS/RSS Case Manager will take on special projects or additional duties as assigned by the Director of RHAS/RSS.
Competency Requirements
- Ability to speak, read, write, and understand English
- Ability to effectively communicate in Spanish (required)
- Ability to assess appropriate matches for potential placements
- Ability to effectively communicate and interact with others
- Must have excellent oral communication skills
Education and Experience
- 32 units from an accredited university, college, or community college with at least 16 units in social work, social science, humanities, public administration, or public health.
OR
- Two-year minimum experience in the field of social work, behavioral science, or social science.
Certifications/Licenses
- Valid CA driver’s license
- CPR/First Aid
Additional Requirements
- Tuberculosis clearance
- Department of Justice/Fingerprint clearance
- Satisfactory completion of health screening
- Proof of legal eligibility to work in the United States
- Satisfactory driving record

While performing the duties of a Relative Home Assessment Services, the employee is frequently required to sit, walk, drive stand, and use their fingers to manipulate a keyboard or writing instrument. The employee occasionally reaches. The employee is rarely required to run, bend, or kneel.
