Responsibilities:
- Under the guidance of the Program Director, design, develop and implement day habilitation service plans that facilitate individual progress
- Complete annual assessments, support strategies, day habilitation support plans, quarterly summaries and other documentation as required by day habilitation regulations
- Support and encourage individuals in meeting their habilitation goals
- Provide direct instruction to individuals.
- Bachelors Degree in related field required, plus two years of experience with individuals with disabilities or in other educational/human service setting Valid driver’s license and insured vehicle
