Social Worker
Position Summary
The Social Worker provides comprehensive psychosocial services to residents, patients, and their families to promote emotional well-being, quality of life, and safe transitions of care. This position conducts psychosocial assessments, develops and implements individualized care plans, coordinates community resources, advocates for residents, and collaborates with the interdisciplinary healthcare team to ensure compliance with federal and state regulations governing long-term care.
Essential Duties and Responsibilities
- Conduct comprehensive psychosocial assessments upon admission and at required intervals, documenting findings in accordance with regulatory and facility requirements.
- Participate in the completion of Minimum Data Set (MDS) assessments, interdisciplinary care planning, and resident progress documentation.
- Develop, implement, and monitor individualized social service care plans.
- Maintain accurate, timely, and confidential documentation within the medical record.
- Serve as a resident advocate by promoting resident rights, dignity, and quality of care.
- Provide emotional support, counseling, crisis intervention, and problem-solving assistance to residents and their families.
- Coordinate resident admissions, transfers, discharges, and referrals while collaborating with third-party providers and community agencies to ensure continuity of care.
- Identify and connect residents and families with appropriate community resources and support services.
- Participate in weekly interdisciplinary care conferences and collaborate daily with nursing staff, providers, and other healthcare professionals regarding resident needs.
- Evaluate and respond to referrals involving behavioral health concerns, substance use, psychosocial needs, and crisis situations.
- Report suspected abuse, neglect, or exploitation of vulnerable adults or children in accordance with state law and facility policy.
- Provide education and in-service training to staff regarding psychosocial issues, resident rights, behavioral health, and related topics.
- Assist with orientation and job-specific training of departmental staff as assigned.
- Support community outreach, educational programs, and facility marketing initiatives as requested.
- Serve as a liaison between Boulder City Hospital, residents, families, community organizations, and external agencies.
- Maintain compliance with all applicable federal, state, accreditation, and facility policies and regulations.
- Perform other duties as assigned.
Required Knowledge, Skills, and Abilities
- Knowledge of federal and Nevada state regulations governing long-term care and healthcare social services.
- Strong assessment, counseling, crisis intervention, and problem-solving skills.
- Excellent verbal and written communication skills with the ability to establish effective working relationships with residents, families, physicians, staff, and community partners.
- Ability to function effectively as a member of an interdisciplinary healthcare team.
- Demonstrated ability to maintain confidentiality and exercise sound professional judgment.
- Strong organizational, documentation, and time management skills.
- Ability to prioritize multiple responsibilities in a fast-paced healthcare environment.
- Proficiency with electronic medical records and standard computer applications.
- Commitment to providing compassionate, resident-centered care.
- Ability to read, write, and communicate effectively in English.
Minimum Qualifications
- Current Nevada Social Worker license in good standing.
- Bachelor's degree in Social Work (BSW) or a closely related field. A Master's degree in Social Work (MSW) is preferred.
- Previous experience in long-term care, skilled nursing, rehabilitation, hospital, or other healthcare setting preferred.
Physical Requirements
- Ability to sit, stand, walk, bend, reach, and move throughout the facility for extended periods.
- Ability to occasionally lift and carry up to 25 pounds.
- Ability to travel locally as needed for community outreach or professional responsibilities.
- Ability to meet occupational health and respiratory protection requirements, including respirator fit testing when required.
