Job Description:
The Calvert County Government’s Office on Aging is seeking a Community First Choice Supports Planner who can work independently and be able to contribute as a team player.
This position requires the ability to establish and maintain effective working relationships with coworkers, the public, and community partners; communicate effectively, both orally and in writing; understand the psychological, emotional, and social needs of the elderly and persons with disabilities; and deal effectively with the public.
Responsible for all Community First Choice Supports Planning for Adults, aged 18 and over. The work involves outreach, tracking applicants and assisting with the eligibility process, provider assistance, case management, and budget compliance. This is a medical model program. The work requires a degree and experience in the field. Workers report to a technical supervisor.
Essential Job Functions
Coordinates and tracks the client application process for the Community First Choice (CFC). Contacts may include applicants, family members, involved agencies, providers, and other interested parties.
Provides information and assistance in completing application process and in obtaining additional services.
Prepares care plan; coordinates with participating agencies, to assure plan meets client’s needs and that it is compliant with Medicaid regulations.
Assigns case management to OOA grant funded position or provide ongoing case management services. Locates and arranges for the delivery of appropriate health, social, and community resources.
Maintains confidential files on each applicant and client, meeting CFC requirements for record keeping.
Monitors use of CFC services to ensure delivery of authorized services and client satisfaction.
Monitors expenses to ensure that they are consistent with the care plan and with cost-neutrality requirements.
Ensures budget compliance by maintaining data on CFC billing database.
Assumes responsibility for all statistical reports for the program.
Recruits providers to assure CFC participant access to needed services. Identify and contact prospective providers, provide information, and assist in completion of enrollment application.
Provides outreach to the community to identify potential persons who can benefit from the CFC.
Participates in all meetings and webinars pertaining to the CFC, monthly OOA staff meetings, and other meetings recommended by supervisor. Obtains training applicable to the CFC program.
Provides information and assistance to potential clients, their families, and the public regarding services and resources available to seniors and the disabled.
Transports clients to necessary appointments in county vehicle.
Assists clients and caregivers with durable medical equipment requests and incontinent supplies.

Knowledge, Skills, and Abilities (These are pre-employment KSA that apply only to Essential Job Functions.
Knowledge of –
Principles and practices of social work as applied to the needs of seniors.
Physical, psychological, emotional, social and health needs of the elderly.
Computer skills.
Ability to –
Establish and maintain effective working relationships with the public and others encountered in this work.
Communicate effectively orally and in writing.
Prepare and maintain required forms and reports.
Deal effectively with the public.
Required Qualifications (Note: Any acceptable combination of education, training and relevant experience that provides the above knowledge, abilities and skills may be substituted on a full-time year for year basis.)
Training and/or Education:
Bachelor’s degree in social work or a related field.
Experience:
Three years of experience with case management and/or working with the elderly.
Licenses or Certificates:
Licensed social worker certificate desired.
Valid driver’s license.
Special Requirements:
Subject to background investigation.
Physical Demands:
Requires long periods of standing, frequent walking indoors, repeated bending, crouching, stooping, stretching or reaching; recurring lifting of objects up to 49 pounds; operation of keyboard devices.
Unusual Demands:
Work is subject to frequent interruptions.
FLSA Status:
Non-exempt
Compensation Steps:
Pay rate: $38.22 per hour
Grade H24, Hourly position, up to 35 hours per week
