Position Summary:
The Hospice Staff RN is a key member of the Hospice team, providing skilled, compassionate nursing care to patients in their homes. This role may serve as a case manager and assist with patient admissions. The RN evaluates patients, develops and implements individualized care plans, and provides support to patients and families in accordance with physician orders and hospice philosophy. The nurse demonstrates professional judgment, psychological sensitivity, and ensures all care complies with regulatory and departmental standards.
Essential Responsibilities:
- Conduct patient assessments, develop individualized care plans, and implement interventions
- Provide case management and assist with admissions, including patient and family education on hospice services
- Administer treatments, including IV therapy, and manage central lines as needed
- Document care accurately using electronic health record systems
- Supervise hospice aides assigned to patients when appropriate
- Participate in staff meetings, in-services, and professional development opportunities
- Communicate effectively with patients, families, healthcare providers, and referral sources
- Respond to emergencies, perform CPR, and ensure patient safety
- Travel to patient homes safely, navigating diverse terrain and environments
- Share on-call responsibilities as assigned
- Valid Oregon driver’s license, insurable driving record, and reliable transportation.
Qualifications & Requirements
- Education: Graduate of an accredited school of nursing.
- Licensure: Current, unrestricted Registered Nurse (RN) license in the State of Oregon.
- Experience: Three years of recent medical/ surgical experience or 1 year recent critical care experience. Hospice or Home Health experience preferred
