HOW YOU’LL MAKE AN IMPACT
Mental Health Targeted Case Manager (TCM) provides home and community-based mental health services to adults with severe and persistent mental illness (SPMI), and children with severe emotional disturbance (SED) and their families.
- Establish relationships with assigned clients and supports and meets with each client as planned.
- Assess, refer, coordinate, monitor services and communicate regularly with all providers involved to ensure appropriate care, reduce duplication of services, and promote client autonomy.
- Be familiar with implementation of health integration, including care coordination agency wide.
- Maintain current knowledge of the mental health service system with the ability to location information and resources for a variety of client needs, including medical, housing, employment, social services, and community programs.
- At a minimum meet the monthly required contact for each client on caseload; document all services provided accurately, completely and within required deadlines.

- Valid Driver’s license and acceptable driving record. Complete initial and ongoing training requirements defined for the position to meet required standards.
