COMPENSATION
Hourly Pay Range: $67.593044 - $86.001866
- After six (6) months of satisfactory employment, employees are eligible for a 3.5% merit increase.
- Eligible newly hired employees will have the one-time option to frontload their first year of vacation accruals (see full details below).
- Employer-paid CME funds and dedicated paid time for continuing medical education and professional development.
Salary offers will be made within the posted pay range and will be based on a candidate's experience (paid or unpaid) that is directly relevant to the position.
JOB DETAILS AND QUALIFICATIONS
We are looking for a Nurse Practitioner passionate about healthcare who is committed to working in a collaborative, team-based environment to join Clackamas County's Health clinic teams at our Sandy Health Clinic!
Clackamas Health Centers (CHC) provides fully integrated care to its patients in a dynamic, team-based environment. Our team of primary care providers are supported by a strong team of RNs, pharmacists (dispensing and clinical), behavioral health providers (including a strong primary care based MAT team), and dentists. We are looking for a family nurse practitioner passionate about caring for the underserved at our Sandy clinic. This position is a growth role- in other words, you get to build your own panel! This position will care for all ages, with more of a focus on adults. A strong interest in providing care for opioid use disorder is a plus.
Required Minimum Qualifications/ Transferrable Skills:*
- Licensed as a Registered Nurse by the State of Oregon.
- Licensed and Board Certified as a Nurse Practitioner with prescriptive privileges (within 30 days of hire) by the State of Oregon.
- DEA registration certificate with Schedule II–V privileges (within 30 days of hire).
- Must possess and maintain a Basic Life Support (BLS) certificate.
- Experience as a Nurse Practitioner in a family medicine setting, with a focus on providing comprehensive primary care services.
Preferred Special Qualifications/ Transferrable Skills:*
- Experience working in an FQHC setting.

- Must pass post-offer, pre-employment drug test.
- Must pass a criminal history check which may include national or state fingerprint records check.

TYPICAL TASKS
Deliver Comprehensive Primary Care:
Provide full-spectrum primary care services to patients of all ages within a fully integrated health center. Care includes diagnosis, treatment, and prevention of acute and chronic illnesses, with appropriate referrals to specialty care and community resources when needed.
Perform Diagnostic Assessments:
Order, interpret, and evaluate a wide range of laboratory tests, imaging studies, and other diagnostic procedures. Use clinical judgment to formulate treatment plans based on findings and patient history.
Prescribe and Manage Medications:
Prescribe medications in accordance with established formulary, professional standards, and clinical guidelines. Monitor patient responses to medications and adjust treatments as needed. Educate patients on medication use, potential side effects, and adherence.
Provide Pediatric Services:
Conduct regular wellness and physical examinations for infants and children. Monitor growth and development milestones, diagnose and treat common pediatric conditions, and ensure children receive timely immunizations. Offer guidance to parents on nutrition, behavior, and developmental concerns.
Offer Prenatal and Reproductive Health Services:
Provide routine obstetric (optional) and gynecological care, including prenatal exams (optional), contraception counseling, STI testing and treatment, and menopause management. Educate patients on reproductive health, pregnancy, family planning, and parenting.
Participate in Clinical Operations and Quality Improvement:
Contribute to the development and review of clinic policies, procedures, and care protocols. Participate in clinic meetings, quality assurance activities, and initiatives that improve service delivery and patient outcomes. Engage in ongoing professional development and in-service education.
Collaborate with Interdisciplinary Teams:
Work collaboratively with behavioral health providers, nurses, case managers, and administrative staff to ensure coordinated, patient-centered care. Utilize internal consultation resources for complex cases.
WORK SCHEDULE
This position works 32 hours during a standard workweek of Monday through Friday (4 days/week; 8 hours/day). Specific hours of work will be discussed with the candidate selected for this position at the time an offer of employment is extended.
This position is designated as on-site/in-person. It is expected that the selected candidate will perform duties at the specified on-site work location.

Employees must work a minimum of 30 hours per week to receive full benefits. Employees working 20 - 29 hours per week are eligible for medical benefits for themselves and family members.
