ABOUT US:
Community Health is a multi-site FQHC serving 80% of the primary care needs in our community (central Vermont).Our mission is to service our patients with quality, efficiency, accessibility and equity. As the largest FQHC in northern New England, we have a large footprint that provides us with the opportunity to influence the evolving, local, state and regional health care environment.
Community Health Center sites are certified Level 111 Medical Homes, with a highly developed and supportive panel management and case management system in place. Community Health also has a robust integrated behavioral health program employing Psychiatrists, Behavioral Health Nurse Practitioners and Licensed Clinical Social Workers embedded within its primary care sites. We have a fully integrated electronic medical record and practice management system (Medent) with robust interfaces with our local hospital.
POSITION SUMMARY:
Community Health is seeking a Nurse Practitioner or Physician Assistant to work at our Rutland, Castleton and Brandon clinics. Candidates are expected to be knowledgeable and skilled in the delivery of primary care. Responsibilities include, but are not limited to:
- Obtains complete medical history and physical examination on patients.
- Reviews medical records.
- Interprets and integrates data to determine appropriate diagnostic and therapeutic procedures.
- Establishes presumptive diagnosis.
- Orders appropriate laboratory and diagnostic procedures.
- Develops a therapeutic plan for patient.
- Sutures wounds.
- Prepares written prescription orders for drugs and controlled substances.
- Instructs patients and family regarding medications and treatment instructions.
- Provides recommendations regarding health and illness prevention.
- Recommend community resource to meet patient/family needs.
- Performs independent evaluations and treatment procedures in emergency situations.
- Provides monitoring and continuity of care between physician visits.
KNOWLEDGE, EDUCATION AND SKILL REQUIREMENTS:
- 3-5 years Primary Care Practice Experience.
- Graduate of an accredited Acute Care Nurse Practitioner or Physician Assistant Program.
- Licensed for Nurse Practitioner or Physician Assistant license in the state of Vermont.
- Strong inpatient skill set.
- Quality-focused team player.
- Excellent organizational, time management and problem-solving skills.
- Ability to function effectively as part of a multi-disciplinary team.
- Effective oral and written communication skills.
- Commitment to providing exceptional patient care.
