We are seeking a Raise the Age Case Manager to provide case management services to individuals eligible for Raise the Age services and that meet the criteria set forth by Erie County as an Adolescent Offender. Intensive case management includes facilitating groups, delivering comprehensive assessments, developing service plans, oversight of workforce readiness groups and linkage.
POSITION RESPONSIBILITIES
1. Completes intake, assessment, and enrollment tasks based on programs standard operating procedures as assigned by the Senior Case Manager.
2. Develops non-criminogenic service plans that are rooted in all service care models outlined in the program manual and submits to Senior Case Manager for final approval.
3. Maintains the Case Plan with periodic reassessment of the person’s needs, progress towards meeting their goal, and documents any changes needed. Notifies Senior Case Manager for their review and approval within the guidelines determined by the contract.
4. Assists people receiving services and monitors outcomes.
5. Provide advocacy and act as a liaison between participants and juvenile justice entities (attorneys, judges, probation officers and police department personnel).
6. Ensure congruencies for all empirically based practices including the Carey Guides, Moral Recognition Therapy, CLSA and YASI.
7. Provides guidance and services to people as specified in the person’s Case Plan, including independent living skills and workforce development.
8. Arranges for and/or provides transportation and/or accompanies the person on individual appointments to provide advocacy.
9. Makes referrals and linkages to appropriate health, counseling, housing, and transportation services and any other necessary supports.
10. Conducts community outreach and education as assigned.
11. Documents all case management activities contemporaneously.
12. Facilitates, completes and maintains required individual records, including case notes, assessments, billing and any required paperwork in compliance with Erie County regulations.
13. Files and processes Incident Reports and ensures appropriate interventions and follow-up as needed.
14. Ensures compliance with the associated Raise the Age proposal.
15. All other duties as assigned.
SUPERVISORY RESPONSIBILITIES
This position has no supervisory responsibilities.
1. Education and Experience:
a. Bachelor’s degree in child & family studies, community mental health, counseling, psychology, rehabilitation, social work, sociology, or criminal justice, and
b. Two (2) years of experience providing direct services or linkage services to person’s diagnosed with mental disabilities, developmental disabilities, alcoholism or substance abuse, OR
c. A minimum of a Bachelor’s degree in any field, and
d. Five (5) years of experience working directly with persons with behavioral health diagnoses; OR
e. A Credentialed Alcoholism and Substance Abuse Counselor (CASAC), and
f. Two (2) years of experience providing direct services or linkage services to persons diagnosed with mental disabilities, developmental disabilities, alcoholism or substance abuse.
2. Skills, Knowledge, and Abilities:
a. Knowledge of developmental disabilities.
b. Good oral and written communication skills and basic computer skills.
c. Working knowledge of community-based services for the population.
d. The ability to read and write case notes as well as understand written directions.
e. The ability to organize, plan and manage caseload with flexibility, dependability, discretion, and independent judgment.
3. Verifications and Screenings:
a. DOH and OPWDD Screening for convictions of criminal offenses.
b. Use of a personal vehicle.
c. Proof of being an insured driver of personal vehicle.
d. A valid NYS Driver’s License.
e. Approval of driving record according to Agency Policy and Procedure.
f. Approval of the NYS Justice Center.
g. Clearance through the NYS Central Child Abuse and Maltreatment Register.
h. County clearance of criminal convictions.
i. A drug screen prior to the commencement of employment.
j. Tuberculosis screening started or completed prior to the commencement of employment.
4. Physical and Mental Requirements:
a. Seeing, hearing, walking, sitting, standing, and driving.
b. Exercising independent judgment, concentrating, thinking, communicating, interacting with others, managing multiple tasks and coping with stressful situations and deadlines.
c. Staying awake and alert while on duty.
WORKING CONDITIONS:
1. Will work in office environment, typically during business hours. May need to work outside of business hours to accommodate appointments or a person’s needs.
2. Frequent local travel required.
