Why is this Role Essential?
The Lead Social Worker serves as a regional clinical and operational leader, providing direct patient care while overseeing Social Workers within their assigned region. This role ensures compliance, staff development, process implementation, and consistent delivery of high-quality psychosocial services across all assigned units. The Lead Social Worker acts as a liaison between frontline Social Workers and leadership teams, promoting operational excellence, regulatory compliance, and patient-centered care. Based on the existing Social Worker role, which reports to market leadership and supports psychosocial care planning and compliance, the SW Lead expands these responsibilities to include staff supervision and regional oversight.
What Will You Do?
1. Leadership & Staff Supervision
- Supervise and support Social Workers within the assigned region.
- Conduct recurring one-on-one meetings with direct reports at least monthly.
- Host monthly or quarterly Social Worker team meetings.
- Conduct annual performance evaluations.
- Provide coaching, mentorship, and performance feedback.
- Conduct disciplinary meetings and corrective action discussions when necessary.
- Ensure direct reports maintain required competencies, annual training, and Concerto University requirements.
- Review and approve employee timecards, mileage reimbursement, and other approved expenses.
2. Regional Operations & Program Oversight
- Maintain an active direct patient care caseload of approximately 50–75 patients.
- Create and manage monthly unit assignments for Social Workers.
- Ensure smooth implementation of new programs, workflows, and operational initiatives.
- Troubleshoot regional issues and escalate concerns to management as appropriate.
- Attend bi-weekly SW Lead meetings and other leadership forums.
- Cover units, patient care responsibilities, or meetings as operational needs require.
- Obtain and maintain out-of-state licenses when necessary to support regional coverage.
- Coordinate staffing and social work support for new unit openings and closures.
3. Recruitment & Workforce Planning
- Assess staffing needs and identify vacancies within the region.
- Submit and monitor job requisitions through Paycom.
- Participate in interviewing, onboarding, and training of new Social Workers.
- Coordinate staffing plans for new den openings and expansion projects.
4. Quality Assurance & Compliance
- Perform quarterly chart audits and compliance reviews.
- Review End-of-Month reports submitted by direct reports.
- Collect and submit monthly caseload reports for assigned team members.
- Monitor documentation compliance and adherence to CMS, state, and company standards.
- Attend monthly QAPI meetings and other quality-focused initiatives.
- Review physician compliance regarding CIPA completion and participation in Plan of Care (POC) meetings.
- Ensure appropriate follow-up and documentation of AKI patients in Salesforce.
- Monitor 2728 completion status and provide support to Social Workers as needed.
5. New Unit Openings & Facility Support
- Participate in planning meetings for new unit openings.
- Coordinate social work staffing and service implementation for new facilities.
- Oversee PHI processes and obtain required laboratory information during unit openings.
- Coordinate and attend facility in-services related to new den openings.
- Assist with patient placement and continuity of care during unit closures.
6. Patient Advocacy & Escalation Support
- Provide guidance and support for patient grievances and complaint resolution.
- Assist with complex patient behavioral concerns.
- Support management of potential involuntary discharges.
- Collaborate with Renal Network representatives and interdisciplinary teams as needed.
- Assist Social Workers with facilitating Plan of Care meetings when attendance barriers exist.
- Serve as a regional resource for complex psychosocial cases, ethical concerns, and patient advocacy issues.
7. Interdisciplinary Collaboration
- Partner with Operations, Nursing, Physicians, and Facility Leadership to ensure coordinated patient care.
- Promote effective communication between Social Workers and interdisciplinary team members.
- Support regional initiatives focused on patient outcomes, regulatory compliance, and quality improvement.

What’s Required?
- Master’s degree in social work.
- Healthcare or dialysis related background preferred.
- Must hold a valid state license; clinical not required.
- Unencumbered driver’s license.
- Intermediate computer skills and proficiency with MS Word, Excel, Outlook as well as becoming proficient with Concerto specific applications within 60 days of employment.
Physical Requirements & Work Environment
The role requires the ability to lift at least 20 pounds and maintain strong mental, visual, and auditory focus when interacting with patients, staff, and customers. The work environment is typical of a healthcare facility, featuring controlled temperatures and moderate noise levels, with potential exposure to infectious or contagious materials. Additionally, the position involves travel between different facilities, necessitating flexibility and mobility.
