About the Role
We are seeking a compassionate, organized, and detail-oriented Care Coordinator to join our Care Management team. This role is closely with medical providers, behavioral health providers, complex care managers, and interdisciplinary team members to support the coordination of comprehensive, patient-centered care.
The ideal candidate is a proactive professional with strong communication, organizational, and problem-solving skills who thrives in a collaborative healthcare environment. This position provides essential clinical and administrative support by coordinating referrals, assisting with care management activities, maintaining accurate client data, connecting patients with community resources, and helping address entitlement, housing, and other social service needs. Through collaboration and effective care coordination, the Care Coordinator plays a vital role in improving patient outcomes and supporting the organization's mission of delivering high-quality, integrated care.
Key Responsibilities
- Interacts with patients and patient caregivers as appropriate to ensure continuity of care, patient adherence to care plans, and identification of barriers preventing adherence to care plans.
- Coordinates care with other members of the care team to overcome any identified financial or social barriers that inhibit patients’ medical care.
- Provides care coordination/management services to individual clients, groups of clients, and/or families in the CS-HHC setting or general community, in a manner that is consistent with professional standards
- Assists client and care management team in obtaining and maintaining insurance, entitlement and other resource authorizations as needed to provide core health and behavioral health and referral services
- Works with care management team to identify client/patient need for advocacy in the area of accessing health and behavioral health systems and services, entitlements and community resources
- Performs all other duties as assigned by the Director of Care Coordination.
Required Qualifications
- High School Diploma/GED required. Associate or bachelor’s Degree preferred. Care Management, CHW (Community Health Worker) Certification(s) preferred and/or required for designated roles.
- Human Service/Health Care experience is preferred.
- Excellent working knowledge of New Haven community social and health service agencies is preferred.
- At least one (1) year experience working with relevant population (employment and/or internship may qualify as experience).
- Computer skills, including data processing and e-mail abilities are required.
- Excellent health care support and organizational skills are required to maintain the work/caseload.
- Knowledge of and demonstration of professional ethics are essential.
- Ability to work well with multi-disciplinary service providers, and to take direction.
- Must demonstrate age specific/population specific competencies and cultural competence.
- The ability to work independently as well as with supervisory direction is very important.
