The Public Health Department promotes the health of the entire community through varied programs ranging from reproductive health care and coordination of the response to health emergencies to individual case management to primary prevention of disease and wellness promotion. We are currently seeking one Public Health Nurse II to be an integral part of our immunization team. This PHN is assigned to Client Care Services to provide education and outreach to healthcare providers, schools, daycare providers and residents of Nevada County in order to increase the community immunization rates.
EXAMPLE OF DUTIES
This Nurse will partner with local coalitions and community partners when possible.
- Will provide immunizations based on best practice ACIP/CDC standards.
- Plan, implement and provide immunization services within the community to vaccinate high-risk populations, underserved communities and ensure safety-net for vaccine access.
- With the Immunization team, administer programmatic scope of work, including collaboration and planning meetings, state reporting requirements, and guidance of state and federal immunization laws.
- Facilitate and ensure County and community partner use of the California Immunization Registry (CAIR).
