Job Description:
A. POSITION SUMMARY:
Ensure the delivery of comprehensive care coordination of Behavioral Health services to Desert Sage Health Centers’ clients and their families. Responsible for accurate and complete documentation of service to clients and families. Works as part of the Behavioral Health team and is responsible for contacting clients by letter, and phone to address follow-up care. Collaborates with other health team members assisting in the promotion of the health and well-being of patients.
B. SUPERVISION RECEIVED:
Work under the daily supervision of the Behavioral Health Clinic Manager.
Responsible to report and work under the Behavioral Health Clinic Manager according to Desert Sage Health Centers’ policies, in the absence of the Behavioral Health Clinic Manager report to the Behavioral Health Director.
C. PRIMARY DUTIES AND RESPONSIBILITES
Coordinates, schedules, and provides registration support to Behavioral Health (BH) Providers.
Coordinates with the BH Director and BH Clinic Manager to ensure productivity goals are consistently met.
Assist in implementation and maintenance of BH protocols to provide prompt access to BH services by ensuring appropriate referrals and efficient triage/scheduling of referred BH clients.
Conducts behavioral health intake and registration processes for new and established clients, ensuring completion of required documentation prior to assessment appointments. Educates clients on intake forms, privacy practices, patient rights, financial assistance programs, releases of information, and insurance requirements. Collects, verifies, and enters demographic, insurance, eligibility, and required FQHC reporting data into the electronic health record, ensuring accuracy and compliance with organizational, state, and federal requirements.
Collects time of service co-payments, accurately maintains a cashbox, and completes daily cash
reconciliation reports.
Updates and verifies demographic information for established clients to include: addresses, phone numbers, insurance benefits, and emergency contact. Check for missing information and collect if appropriate.
Assists Behavioral Health Scheduler with prompt check-in of clients arriving for individual and group appointments, while monitoring wait times.
Participate in morning huddles with entire BH Staff to prepare for workday to include need for interpretation, available appointments, triage and ensuring that schedules are at capacity for each day and the next.
Direct medication refill requests and other medical inquiries to the clinical support staff via a patient case in the electronic health computer system.
Maintains the BH specialty wait list, BH school-based counseling wait list for accuracy and access to care.
Coordinates school-based counseling clients with the school-based counseling team and school district staff on an on-going basis.
Supports BH Admin staff, Behavioral Health and Medical Providers to deliver seamless integrated behavioral health services in the primary care setting.
Maintain strict confidentiality on all patient information gained in the DSHC practice setting of this position. Treat all clients with dignity & compassion without creating barriers.
Maintains quality of care in accordance with the Continuous Quality Improvement plan.
Participate in program and service delivery planning, development, implementation, quality improvement projects, and evaluation relating to patient care and clinical processes.
Participate in staff meetings, conferences, outreach events, and case management activities.
Utilizes interpreters to converse in Spanish with DSHC clients as necessary. Maintain awareness and acceptance of cultural competency aspects and sensitivity.
Collaborate with internal and external referral coordinators to coordinate client care.
Clean and maintain workspace, computers, printers, and other office equipment on a regular basis according to equipment maintenance procedures.
Assist in training new Behavioral Health Staff as necessary
Employee knows DSHC’s 5 core values and demonstrates the specific valued behaviors on a daily basis.
Perform all other duties as assigned and necessary for proper operation of the health center.
D.PREFFERED QUALIFICATIONS:
Baccalaureate/Associates degree in Public Health or in appropriate health field or a combination of education and experience preferred.
Experience with outreach activities preferred.
Knowledge and experience with case management approach.
Experience with interagency coordination.
Experience with electron healthcare records systems
Experience with medical front office scheduling
Bicultural/bilingual or experience working with Hispanic population.
E. TECHNICAL SKILLS:
Strong verbal communication skills. Courteous and empathic personality.
Ability to operate electronic health computers/keyboard and phone systems.
Ability to work under pressure and handle multiple tasks.
Ability to maintain confidentiality per the Privacy Act.
Possess good judgment about handling clinical emergencies and behavioral problems.
TYPICAL PHYSICAL DEMANDS
Position requires prolonged sitting, walking, standing, frequent bending, stooping or stretching. Requires corrected vision and hearing to normal range, full range of body motion including manual and finger dexterity and eye-hand coordination May be exposed to communicable diseases and bodily fluids and/or stressful conditions with patients who may be in discomfort and/or pain. Requires ability to sustain concentration in a noisy and fast-paced environment.
