Position Overview
Holy Family Prenatal Care is seeking a compassionate and dedicated Social Worker with LSW or QMHS certification to join our team. The ideal candidate will be a strong advocate for patients, skilled in identifying patient needs, making appropriate referrals, and providing follow-up support. This role requires a reliable, confident professional who fosters good working relationships and demonstrates a strong personal motivation and sense of responsibility. A servant’s attitude and the ability to work collaboratively as a strong team player are essential to support our mission of providing exceptional prenatal care.
- Our primary purpose is to protect unborn life.
- We believe in the sanctity of marriage between one man and one woman and the sacredness of the marital act.
- We promote Natural Family Planning (NFP) and we do not promote any form of artificial contraception or sterilization.
- We promote abstinence before marriage and fidelity within marriage.
Key Responsibilities
- Identify patient needs and provide appropriate counseling and support.
- Make timely and appropriate referrals to community resources and services.
- Complete follow-up to ensure patient needs are met and services are effective.
- Maintain strict client confidentiality in all interactions and documentation.
- Advocate for patients’ rights and well-being within the healthcare setting.
- Collaborate effectively with healthcare providers and team members to support patient care plans.
- Encourage and motivate patients to achieve their personal and health-related goals.
- Support the mission and values of Holy Family Prenatal Care through a servant’s heart and professional attitude.
Required Skills and Qualifications
- Licensed Social Worker (LSW) or Quality Mental Health Specialist (QMHS) certification.
- Proficient in Google Workplace applications.
- Strong understanding of client confidentiality and ethical social work practices.
- Reliable, confident, and able to build and maintain good working relationships.
- Strong personal motivation and a clear sense of responsibility.
- Servant’s attitude with a commitment to supporting patients and the team.
- Excellent teamwork skills and ability to collaborate effectively.>
- Patient advocate with excellent counseling and communication skills.
- Goal-oriented with the ability to encourage and support patients in achieving outcomes.
- Experience with EPIC or other EMR system preferred
