Be the First Call That Makes a Difference: Join Us as a Triage Nurse (RN)

Faith Hospice is seeking an experienced and compassionate RN to join our team as a full-time Triage Nurse. This rare opportunity is onsite, Monday through Friday to serve as a critical first point of clinical contact for patients, families, caregivers, referral partners, and staff, providing timely assessment, clinical guidance, and care coordination through telephone and electronic communication.

Using advanced clinical judgment, the Triage Nurse evaluates patient needs, prioritizes responses, supports symptom management, and coordinates appropriate interventions to ensure safe, effective, and compassionate care. This role plays a key part in enhancing the patient and family experience, supporting continuity of care, reducing unnecessary emergency department utilization, and ensuring that patient needs are addressed promptly and appropriately.

Schedule

This role is based onsite at the Faith Hospice main office in Byron Center, Michigan working a Monday – Friday daytime schedule.

Education and Experience

• Licensed Registered Nurse (RN) required; unrestricted nursing license in the state of Michigan
• Minimum of 2 years of nursing experience required
• Hospice, palliative care, home care, emergency department, call center, or triage nursing experience preferred.
• Hospice and palliative care experience strongly preferred
