Position will serve as the facilitator of the child and family teams, advocate for families in the program, provide linkage with needed community resources, assist in treatment and education to families and provide crisis support and safety planning.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned
Adheres to agency policy and procedures, as found in the online SharePoint Folder. Also adheres to CARF and DMHSAS standards and criteria, federal and state confidentiality rules and regulations, and any special funding source requirements including Medicaid and HCA regulations.
Facilitates implementation of the phases and activities of wraparound and service coordination. Ensures the team is moving the children, youth and family toward long-term stability through the wraparound process and service coordination.
Ensures all team members contribute and are active partners in the wraparound process.
Partners with all professional and natural supports on the family team to promote appropriate and integrated services.
Respects the integrity and confidentiality of the persons receiving wraparound services.
Conducts strengths, needs, cultural discoveries and crisis plans for each family. Prepares written report and gives to family according to the wraparound model.
Develops strength-based family guided wraparound plan for each child and family team based on their individual needs.
Coordinates and facilitates the wraparound process with the child/youth and family team to keep children/youth in the their schools, home and avoid out of home placements of the children/youth.
Empowers the child, youth and family through general modeling, advocacy, coaching, follow up, and support. Teaches the children, youth and family how to effectively advocate within various child-serving agencies to include DHS, Juvenile Justice and the education system.
Develops and maintains positive relationships with community partners. Serves as a liaison between Systems of Care and other community services and groups as directed.
Participates in community consultation and educational activities. Promotes child, youth and family involvement at the individual and community levels.
Monitors eligibility status and makes application for Social Security, Title XIX (Medicaid), QMB and other benefits the client may qualify for and seeks to assist eligible clients in the Health Home Services.
Maintains up-to-date address, phone number, contact persons, and financial information on all clients.
Assists clients in finding and accessing community resources needed to maintain independent functioning.
Assists clients, individually and in groups, with development of independent and community living skills, social skills, interests and leisure time activities which will encourage community involvement and integration.
Assist clients in developing an individualized recovery plan that focuses on personal and interactive problems, independent living skill needs, and defining goals and to plan action reflecting interests, abilities and needs.
Responsible for maintaining strictest confidentiality guidelines for interactions regarding consumer and family information.
Provides a minimum of 85 hours billable services with consumers per month.
Utilizes the philosophy of Collaborative/Therapeutic Documentation to meet clinical record keeping documentation standards and ensure documentation is completed with the Consumer to the extent possible and all documentation is completed on the same day of the date of service.
Responsible for seeking out supervisory assistance for additional training and to enhance skill set as needed.
Interacts with other professionals to discuss therapy or treatment, new resources or techniques and to share information.
Reports and documents services in accordance with agency policy and procedures. Adheres to HOPE policy and procedure of having documentation completed by the end of the next business day.
The work schedule for this position may include working late some days (e.g. 10am-7pm) to accommodate family schedules.
Must be able to provide services in a community setting.
Must be able to provide client transportation using personal vehicle or agency owned van or small bus.
Performs other related duties as assigned.

CERTIFICATES, LICENSES, REGISTRATIONS
Required to obtain and maintain DMHSAS CM II Certification.
Required to complete Therapeutic Options or approved safety training and annual updates.
May be required to obtain and maintain First Aid and CPR Certifications.
In order to provide client transportation ,must possess current, valid Oklahoma license, meet HOPE insurance requirements. HOPE will annually obtain copy of Motor Vehicle Record from the Department of Public Safety.
LANGUAGE SKILLS
Ability to read and comprehend simple instructions, short correspondence, and memos. Ability to write simple correspondence and clinical writing/documentation in medical records chart. Ability to effectively present information in one-on-one and small group situations to families, clients, and other employees of the organization.
REASONING ABILITY
Ability to apply commonsense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations. Ability to seek out supervisory assistance for additional training when needed and make application of techniques or other skills to meet essential job duties and responsibilities.
OTHER SKILLS AND ABILITIES
Ability to relate to persons with severe mental illness and/or substance use in a positive, helpful, and constructive manner.
Ability to establish and maintain effective and tactful working relationships with co-workers; to express ideas and facts clearly and concisely in written or oral form.
Personality and mature presentation of oneself demonstrating the ability to work with various systems while maintaining objectivity and cooperative attitude.
