The Case Manager functions under the direct supervision of the Director of Social Services. He or she assumes responsibility for working with the treatment team and the patient in order to assist in basic discharge planning needs, coordination of aftercare and placement of the patient. This includes residential needs, aftercare needs, advocacy needs, entitlement needs, and building relationship with aftercare providers. Other duties as assigned by supervisor.

Education/Training:
- Bachelor’s Degree in psychology or related field.
- Crisis Prevention Intervention (CPI) required or equivalent
- CPR
Experience:
A minimum of two (2) years direct clinical experience in a psychiatric or mental health setting, with direct experience in case management patient assessment, patient aftercare coordination, and communication with external review organizations. Experience in working with community resources, residential/outpatient programs and outside referrals.
Knowledge and Skills:
Extensive knowledge of community resources and FLBHC’s referral sources in order to make appropriate referrals.
- Ability to communicate effectively verbally and in writing with patients, families, resources, referral sources, community leaders, legal system personnel.
- Ability to work effectively within treatment team disciplines.
- Ability to prepare reports for DCF and other funding sources correctly and in a timely manner when required.
- Knowledge of patient needs assessment, discharge planning, and coordination with family members.
- Knowledge of patient rights, advanced directives, adult and child abuse laws, and involuntary procedures.
- Knowledge of complex patient care planning.
- Excellent leadership and negotiation skills.
- Excellent organizational and interpersonal communication skills.
- Skill in interacting with multiple individuals with diversified roles and perspectives.
- Knowledge of external review organizations (i.e. Managed Care, Medicare)
- Skilled in organizing and prioritizing workloads to meet deadlines.
- Excellent customer relations skills.
- Ability to use good judgment to maintain confidentiality of information.
- Ability to prioritize heavy workloads.
- Sensitive and aware of diverse cultures and their practices.
