Program Description: :
The Apartment Treatment Program, Community Residence is a 54 bed, scattered site program. The program is licensed by the NYS Office of Mental Health and designed for single adults diagnosed with a mental illness who will benefit from a shared living arrangement as they work on the skills that will enable them to successfully transition to more independent supportive housing. Residents have their own bedrooms in scattered sites, two or three-bedroom apartments.
Essential Responsibility and Role of a Senior Visiting Counselor :
The Senior Visiting Counselor has two major areas of responsibility. They are; program responsibilities and reporting responsibilities. The Senior Visiting Counselor is responsible for making scheduled apartment visits, delivering supportive services, and making entries in case records of psychiatric clients who are residents of Geel’s scatter-site apartment treatment program. The Senior Visiting Counselor is responsible for assuming the role of the Unit Director when he/she is absent or when so directed by the Program Director.
Program Responsibilities :
- Assists residents in the acquisition /performance of daily living skills (DLS) that include but are not limited to: personal hygiene/grooming, shopping, food preparation and budgeting.
- Supervises apartment maintenance chores, assisting residents in cleaning when necessary.
- Assists residents in planning and participating in recreational activities.
- Assists residents in acquisition /refinement of interpersonal skills
- Accompanies residents to other service providers, as needed, i.e. Public assistance, Social Security, day programs and medical/psychiatric appointments, etc.
- Provides availability (on rotating basis) for crisis intervention and cell phone responsibility during non-scheduled work hours.
- Reports any clinical crisis/emergencies to Unit Director, Program Director or Clinical Coordinator
- Reports to Unit Director any needed apartment repairs/maintenance issues and follow -ups on issues as needed.
- Performs physical medication checks in apartments on monthly basis.
- Conducts and documents fire drills and safety inspections as scheduled.
- Maintains regular caseload as assigned by Unit Director and documents all contacts with other providers as well as services delivered as indicated in the client’s service plan in a timely and accurate fashion and in compliance with OMH regulations.
- Conducts monthly medication and program checks.
- Assists in the preparation of quarterly Service Plan Reviews and bi-annual Utilization Management Reviews.
- Attends and participates in weekly individual supervision and unit meetings with Unit Director, staff meetings, case conferences, community meetings, and scheduled trainings.
- Performs other related duties as assigned by Unit Director, including but not limited to chart audits and unit log books
- Provides training to new staff members in their unit as directed by Unit Director or Program Director
- Provides direct supervision to Visiting Counselors in their unit as directed by Unit Director or Program Director and documents as supervisory notes
Qualifications and Requirements:
- This position requires that the individual fulfills the requirements of a Qualified Mental Health Professional (QMHP) with a Baccalaureate Degree in Human Services or higher
- Applicant must have one year’s experience as a Visiting Counselor in an Apartment Treatment Program or at least 24 months as a Case Manager in a Supported Housing program serving a similar population
- Bi-lingual Spanish a plus
Physical Requirements :
- Must be able to access apartments above first floor with or without elevator access.
- Must be able travel to different locations using public transportation.
- Must be able to demonstrate proper cleaning and apartment maintenance methods to clients/ residents in addressing their activities of daily living.
