JOB SUMMARY :
The Structural Heart Coordinator will report to the Cardiology Manager. The Structural Heart Coordinator will function as a central figure in the overall operations of the Valve Clinic with specific emphasis on Transcatheter Aortic Valve Replacement (TAVR), and Left Atrial Appendage Occlusion (LAAO) program.

JOB QUALIFICATIONS:
- Minimum of associate degree in health care field preferred or related clinical experience.
- Minimum of 4 years’ experience in cardiovascular care
- Current RN license in good standing
- Basic knowledge of procedures and biomedical products related to cardiology and cardiovascular procedures.
- Experience with data management systems and familiarity with clinical data analysis and reporting.
- Excellent verbal and written communication skills.
- Demonstrated proficiency with office software specifically Microsoft Access, Word, Excel and PowerPoint.
- Flexibility and the ability to work in a dynamic, team-oriented environment.
- Ability to work independently and be self-directed.

DUTIES AND RESPONSIBILTIES :
Program Coordinator(s):
- Coordinates the referral process and introduction of patients into the Heart Valve Clinic for purposes of evaluation and treatment
- Guides and directs the continuum of care for valve patients from admission through discharge, including post-discharge care and follow-up as appropriate for the patient population
- Develops and implements protocols for care of valve patients which are evidence based
- Develops and implements the educational materials and support needed by the patient and family and assures the patients and families receive appropriate educational counseling to assist them in physical and psychosocial progress towards outcomes
- Communicates effectively, both verbally and in written format the progress of the program and identifies areas for opportunity, growth, and improvement to the medical and administrative team on an ongoing basis
Data Registry:
- Coordinate all aspects of TVT and LAAO NCDR Registry. Data Registry functions including data collection, data submission, daily quality report issues, outcomes report review and analysis.
- Assume leadership and facilitation role with facility physicians, staff, and administrative personnel.
ADDITIONAL RESPONSIBILITES:
Program Coordinator(s):
- Receives incoming referral from physicians and hospitals and responds quickly in order to assure rapid access into the system
- Communicates back to referring physicians and hospitals the plans for acceptance into the program in an effective manner
- Educates the patient and family regarding the referral process, steps of evaluation, and keeps patient and family informed throughout the entire process.
- Conducts thorough assessments of the patient as needed (i.e., clinical, psychosocial, surgical risk scoring, frailty, mobility, etc.) required by policy to make an accurate determination of the patient’s qualifications for a given procedure
- Schedules and manages multiple testing according to protocols to complete the assessment process (i.e., cardiac cath, TEE, TTE, etc.)
- Serves as the content expert for valve procedures and assists other staff who help care for these patients as requested to assure that quality care is being delivered in a consistent fashion
- Assists and participates in the coordination and implementation of a post discharge care clinic to evaluate patient progress towards outcomes and identify care issues. Actively identifies solution to process or quality issues to achieve positive outcomes for the patient and family
- Coordinates/ Leads all weekly meetings with multidisciplinary TAVR Team and present all patients who are currently candidates for TAVR to include clinical story, all current imaging results, upcoming testing, potential procedure date and TAVR approach- make sure all TAVR team members are up to date on each patient and where they are in the work-up process – be able to answer any and all questions related to these patients
- Liaison between indicated industry and our TAVR Team Updating all patient studies to Valve Point and Core Valve for review to then be included in patient presentations in our weekly meetings.
- Updates/maintains accuracy of our current Aortic Stenosis patient database- with updated echo results (AV Vmax, Mean gradient, AVA, LVEF), and clinical summary or next steps in TAVR work-up as decided by our team
- Initial or renewal of contracts and rebates.
Data Registry:
- Working in conjunction with IT, physician experts, Quality and Performance Improvement, MHI, and other staff. Coordinate the development and implementation of the TVT and LAAO NCDR Data Registry (Maintaining accurate data and meet reporting deadlines as appropriate,)
- Data Registry outcomes reporting.
- Based on a thorough understanding of NCDR Registry products and services, respond to all requests for information to include:
Clinical interpretation of data element definitions; Data submission process; Data quality and cleaning; Data submission to meet regulatory or stakeholder requirements; Comparative Institutional Report interpretation; and Use of outcome reports for quality measurement and improvement
- Prepare training, as necessary, to assist with quality data collection and interpretation for the TVT NCDR Data Registry
- Coordinate the review, interpretation, and update of Data Quality Reports and Institutional Outcomes Reports.
- Develop and maintain general policies and procedures for data abstraction and submission to the TVT Data Registry

- Participate in NCDR conference calls and webinar training.
- Act as the primary point-of-contact for all administrative tasks relating to the customer website, including but not limited to creating/disabling user accounts and ensuring that the Site Profile is always up-to-date
- Facilitate/coordinate contract and payment inquiries in combination with MHI and Quality.
- Demonstrate the ability to work with autonomy, and the ability to plan, prioritize, and organize.
- Other duties as assigned, including working cases and taking call if the need arises.
