RN Case Manager Support - 36 hours per week, with weekend rotation
SUMMARY OF JOB RESPONSIBILITIES
The Case Manager provides skilled professional nursing care to the patient/family/caregiver as prescribed by the physician and in compliance with regulations as established by the State Board of Registered Nursing, the patient and the policies of Harbor Hospice. Case Manager creates & oversees Plans of Care for each patient admitted to Home Care.

ESSENTIAL FUNCTIONS AND DUTIES (other duties may be assigned)
- Knowledgeable of hospice concept and philosophy.
- Assess patient appropriateness for hospice services. Assess patient/family/care giver and develop plan of care including safety and teaching. Perform GIP on/off audits. Completes LCD at recertification. Prepares for and collaborates in an effective discharge plan.
- Leads weekly interdisciplinary team meetings. Participates in team decisions concerning patients including designating patient acuity. Communicates individually or within a group to keep team advised of patient status.
- Participates in information visits for potential admissions to Home Care
- Assists with admitting new patients and is responsible for ensuring thoroughness of the admission.
- Provides/promotes excellent communication with families, patients and visitors. Follows up with concerns and brings unresolved issues to clinical manager.
- Performs and is knowledgeable of venipuncture and port lab draws; IV starts, and CADD pumps as necessary.
- Knowledge related to use of IV and external feeding pumps; operations and maintenance.
- Completes appropriate charting including daily GIP charting and all RHC for assigned patients. Performs chart audits to meet accreditation and regulatory standards. Maintains visit string updates.
- Follows up with appropriate staff to ensure missing/inaccurate information is completed.
- Manages medication including ordering meds, ensuring compliance, e-box prescription management and ensures proper orders in Home Works.
- Demonstrate proper hand washing, asepsis, bag technique and standard precautions.
- Demonstrate and teach patient/family/care giver proper usage of DME and supplies.
- Provide CENA supervision in accordance to Medicare regulations.
- Orient staff and volunteers as assigned.
- Provide in-service and educational programs to staff and other health professionals as needed.
- Monitors e-box prescriptions, nutritional evaluations, GIP orders, GIP medication pharmacy profiles and signatures, and the Kardex to ensure compliance in these areas.
- Rounds with physician: coordinates appropriate information related to patient’s plan of care.
- Organizes/manages regulatory requirements including: F2F visits due, 60 day visits, and weekend admit notification.

PHYSICAL DEMANDS AND WORK ENVIRONMENT
- While performing the duties of this Job, the employee is regularly required to stand and stoop, kneel, crouch, or crawl.
- The employee is occasionally required to walk; sit; use hands; reach with hands and arms and talk and hear.
- The employee must regularly lift and/or move up to 10 pounds, frequently lift and/or move up to 25 pounds and occasionally lift and/or move up to 50 pounds.
- While performing the duties of this Job, the employee is occasionally exposed to wet and/or humid conditions and outside weather conditions.
- The noise level in the work environment is usually moderate.
QUALIFICATIONS
- Registered Nurse with degree from an accredited nursing school and holds a current license in the State of Michigan.
- Possesses and maintains a current CPR certification.
- Maintains a valid Michigan driver’s license and has the availability of a reliable, licensed and insured vehicle for use on the job, vehicle must be insured in accordance with the State of Michigan.
