About the Role

Heart to Heart Home Care is seeking a highly motivated, tech-savvy Intake & Eligibility Specialist to join our growing team. This role is ideal for someone who enjoys working with technology, solving problems, and helping patients navigate healthcare services.

What You'll Do

• Manage new patient referrals from start to finish.
• Verify insurance eligibility and Medicaid coverage.
• Coordinate with hospitals, physicians, insurance plans, and community partners.
• Utilize multiple software systems to document patient information accurately.
• Track referral progress and proactively follow up to ensure timely admissions.
• Educate patients and families on available home care services.
• Schedule assessments and coordinate with clinical staff.
• Maintain accurate documentation while meeting productivity goals.
• Communicate professionally with internal teams and external partners.

• Healthcare, insurance, call center, intake, or customer service experience preferred.

Preferred Qualifications

• Experience with Medicaid, Medicare, MLTC, MAP, or home care services.
• Knowledge of EMR/EHR systems.
• Bilingual (English/Spanish) is a plus.
