At Help at Home, our IDD Client Service Supervisors work with the client and their team to develop and implement a Person-Centered Individualized Support Plan (PCISP). The Program Coordinator is responsible for coordinating caregiver schedules, maintaining client records, and ongoing communication with the client and their team.
NO ON CALL, NO 24-HOUR AVAILABILITY NECESSARY!
Office hours are Monday - Friday 8:30AM - 5:00PM, but work hours for this role will vary to support client needs.

A typical day includes:
Building and maintaining relationships with clients, family members, caregivers, case managers, and team members through regular check-ins, trustworthiness, reliability, and follow-through
Utilizing a relationship-focused approach to make thoughtful client/caregiver matches
Providing high-quality training to caregivers on client’s PCISP, including coordinating any additional training required for behavior or risk plans
Creating and documenting client schedules based on client need, caregiver availability, and PCISP budget/goals
Attending and participating in quarterly meetings to review client PCISP, design and implement support strategies, and collaborating with the client and their team to reach client goals
Maintaining excellent records, including documentation in EMR, writing and submitting quarterly and annual reports, incident reports, and risk plans
Focusing on growth by admitting new clients based on regional goals
Creating great days and meaningful moments for our clients through dedicated, relationship-focused care
What You'll Bring:
A positive attitude and the desire to work hard, help people, and make lasting relationships
Excellent verbal and written communication skills
Organization skills, with a focus on record-keeping and task-prioritization
Strong independent work ethic and problem-solving ability
Experience working with clients with intellectual or developmental disabilities is a plus
A personal vehicle that is insured and meets state safety/licensure requirements
A valid driver’s license
Clean background checks and pass a drug test.
Education and Experience:
- At least 18 years of age.
- High school graduate or equivalent preferred; May require higher level of education or certification.
- Current PPD, or Chest x-ray if applicable.
- Medicaid, Waiver, or Home Healthcare experience preferred.
- Other Requirements pursuant to state or local rules as applicable.
Management Authority:
- Conducts performance reviews
- Trains other associates
- Directs work of other associates
Physical Requirements:
- up to 100 pounds
- Ability to move or traverse about in offices and/or client homes, including ascending and descending stairs.
- Ability to communicate effectively and clearly with others to exchange information.
Travel Requirements:
- Regular travel on a daily or weekly basis required, even in inclement weather
