We are seeking a highly motivated and relationship-focused Clinical Hospital Liaison to join our team. The Clinical Hospital Liaison serves as the primary point of contact for hospitals, physicians, case managers, discharge planners, and healthcare providers, facilitating seamless transitions for patients requiring post-acute rehabilitation, skilled nursing, memory care, or long-term care services.
This position plays a key role in developing and maintaining referral relationships, evaluating potential admissions, coordinating patient transitions, and promoting the organization's commitment to exceptional clinical outcomes and customer service.

Qualifications
- RN or LPN preferred, but not required for an experienced applicant
- Healthcare, long-term care, rehabilitation, hospital discharge planning, admissions, or healthcare marketing experience preferred
- Ability and willingness to travel up to 60 miles, as needed
- Charismatic, energetic, professional, and customer-service oriented
- Strong understanding of the needs of older adults and post-acute care populations
- Excellent verbal and written communication skills
- Strong relationship-building and networking abilities
- Good working knowledge of computers and electronic healthcare systems
- Ability to effectively interact with patients, families, healthcare professionals, and community partners
- Strong organizational skills with the ability to manage multiple priorities in a fast-paced environment
Essential Responsibilities
- Develop and maintain positive relationships with hospitals, physicians, discharge planners, case managers, and community referral sources.
- Conduct clinical and referral evaluations to determine appropriateness for admission.
- Collaborate with facility leadership, admissions teams, and clinical staff to facilitate timely admissions and transitions of care.
- Communicate effectively with patients and families regarding available services and care options.
- Represent the organization at hospitals, healthcare events, and community outreach activities.
- Monitor referral activity and maintain accurate documentation and reporting.
- Serve as a resource regarding post-acute care services, Medicare, Medicaid, and long-term care options.
- Promote a positive image of the organization through professional communication and exceptional customer service.
