Home Care Network is seeking 2 reliable, compassionate, and highly skilled Pediatric LPNs or RNs with excellent in-home infant/toddler care experience to join our home health agency as Full-Time Private Duty Nurses (PDN) to care for a wonderful pediatric client in Lima, OH (Northwest Ohio).
LOCATION & IMMEDIATE CLIENT NEEDS:

The pediatric patient is a precious <24-month-old infant boy with a loving family also involved in his care.
- Patient is currently residing in NICU until 2 full-time PDNs can be secured for care in his home in Lima, OH.

At least 1 year of pediatric direct-patient experience in each of the following is required:
- Trach care
- Vent care
- G-Tube feedings and care
- Client is dependent on care, and staff will have to push, pull, roll, lift, and transfer client so ability to meet all of the basic job requirements is a must.
- Pediatric RN preferred; LPNs with excellent infant/toddler pediatric home health experience will also be considered.

Recent clinical pediatric care experience is required (i.e., pediatric hospital units, pediatric patient home care, etc.).
- Excluded: clinics/physician offices, general hospital units, behavioral health centers, and LTC facilities.

SCHEDULE:

Total Client Care requires up to 84 hours per week.
- 2 Skilled PDNs are requested, with the potential for a 3rd as PRN / As Needed Backup and/or Respite.
- Ideally, both PDNs would work a Hybrid / Blended Schedule including both Daytime and Evening shifts per week (rather than strictly days or strictly evenings), however "Strictly Days or Evenings" schedules will also be considered so long as days per week can be flexible week to week.

PDN Care Shifts include a mix of the following Daytime & Evening schedules, both consisting of primarily Weekdays + occasional Saturdays, with variation in exact days from week to week):
- Daytime: 8AM - 5PM
- Evening: 4PM - 12AM
- There may be flexibility on exact start and end times.

ADDITIONAL HOURS:
- Due to this client's total dependency on care, Overtime Hours will be accrued and paid at 1.5x your hourly rate.
- There is a potential for OPTIONAL additional weekly hours and/or Respite care - offered at the family's discretion, as needed (not required).
ROLE DETAILS:
- Autonomy in a positive, client-centered culture, with excellent Quality and Compliance support + training for all caregivers and clinicians.

Weekly pay (on Fridays) including paid onboarding and training.

No Requirements for On-Call shifts, Weekends, extensive travel distances, or overtime beyond standard scheduled hours.

Friendly travel expectations: Up to 30-minute client travel radius (from your home) is required. Up to 45- or 60-minute client travel radius (from your home) is highly desired (not required).

Mileage reimbursement between multiple clients is provided.

Starting Pay Rate:
- LPNs: $30 - $32 per hour
- RNs: $40 - $50 per hour
- Compensation is based on employee qualifications and certifications, experience, skill specializations, positive employment history, and overall client & team fit.

To support our growth and the increasing volume of client care throughout Ohio, we are seeking a highly skilled, experienced, accountable, and self-motivated home health Pediatric PDN (LPN or RN) to join our agency and serve this care-dependent patient in Lima, OH (Northwest Ohio) in a full-time capacity.
Qualifications
- Graduate of an accredited school of professional nursing.
- Pediatric RN preferred; LPNs with excellent pediatric experience will be considered.
- Current license to practice as a Licensed Practical Nurse/Registered Nurse in the state of Ohio is required.
- This case requires recent clinical pediatric care experience (i.e., pediatric hospital units, pediatric patient home care, etc.).
- 1+ year pediatric G-tube feedings and care experience and knowledge required.
- 1+ year pediatric Trach care experience and knowledge required.
- 1+ year pediatric Vent care experience and knowledge required.
- Client is dependent on care, and staff will have to push, pull, roll, lift, and transfer client so ability to meet all of the basic job requirements is a must.
- Home Health Nursing experience/ familiarity highly desired.
- Active CPR and First Aid certificates highly desired (for speed to start care), but agency will provide recertification if required.
- Good interpersonal communication skills - written and verbal.
- Desirable character traits: reliable, honest beyond reproach, professional, compassionate, detail-oriented (in care + documentation), accountable, friendly and accommodating, respectful.
- Current driver's license or state ID, plus reliable transportation, are required.
- Excellent attendance record with past employers.
Position Summary
A successful PDN at Home Care Network, Inc., performs skilled nursing functions in accordance with the federal, state, and local laws and within the guidelines of his/her professional organization and agency. All functions shall be performed in accordance with the established policies and practices and state Nurse Practice Act. The LPN/RN delivers care to clients as delegated by the RN Case Manager.
Reports to: RN Case Manager; NW Ohio Regional Administrator / Director of Nursing
Essential Functions/Areas of Accountability:
- Provides professional nursing care under the direction of a Registered Nurse. Communicates appropriate status/condition changes to the RN per agency guidelines.
- Provides skilled nursing visits for clients as directed by the RN Case Manager. Follows the care plan established by the Registered Nurse.
- Recognizes and reports changes in client condition to the Case Manager and physician, as directed.
- Communicates information on a timely basis, consistent with agency policy and urgency of the situation. Communication is verbal, as needed, and through complete and accurate documentation in the clinical record.
- Scope of clinical practice includes personal care services, assessment of findings, medication monitoring and administration, client teaching, emergency intervention, and nursing services within professional scope of practice and as delegated by the Case Manager.
- Individuals with the license designation of "LPN M-IV" may perform venipuncture for the purpose of obtaining blood samples for laboratory analysis.
- Promotes personal safety and a safe environment for clients and coworkers.
- Demonstrates knowledge of safety/infection control practices by compliance with policies and procedures.
- Recognizes and responds appropriately to potentially unsafe situations.
- Demonstrates safe practice in the use of equipment.
- Makes recommendations on improvement of safety.
- Performs job in compliance with agency policies and procedures and professional and community standards.
- Accepts responsibility for professional nursing role.
- Follows applicable state and federal laws and agency standards.
- Attends all meetings as required.
- Maintains confidentiality in all aspects of the job.
- Refrains from discussing client or staff identities, conditions, or circumstances in public or with others who do not have a legitimate interest in the person.
- Participates in the development, implementation, and evaluation of the Agency Quality Improvement Program and pertinent activities.
- Performs other related duties and responsibilities as deemed necessary.
