What You’ll Do:
- Oversee the delivery of exceptional nursing care by conducting health assessments, medication administration, and providing wound care and education to clients and families.
- Develop, implement, and evaluate personalized care plans tailored to each person’s health needs.
- Take and monitor vital signs and ensure delivery of treatments for optimal client care.
- Supervise, mentor, and train nursing staff and caregivers, fostering professional development and adherence to state regulations for skilled and non-skilled services.
- Conduct new client assessments and quality assurance visits to ensure compliance with the highest quality care standards.
- Collaborate with multidisciplinary teams, including physicians, therapists, and social workers, to provide comprehensive person-directed care.
- Maintain accurate documentation of care activities.
- Organize and lead regular training sessions for nursing staff to enhance knowledge of home care best practices.
- Support the hiring, evaluation, and retention of skilled nursing staff and caregivers that align with company values.
- Address client and family concerns with empathy and professionalism while leading care coordination meetings.
- Assist in marketing the company’s skilled nursing services to health professionals and maintain relationships with referral sources.
- Ensure compliance with state and federal regulations governing home health services and participate in agency compliance assessments.
- Manage relationships with national accounts.
- Ensure state regulations and CHAP accreditation requirements are implemented, managed, and in compliance.
- Other duties as assigned.
Are You the Right Fit?
- Valid RN license in the state of New Jersey.
- Minimum of 3 nursing experience, with at least 2 years in home care and demonstrated leadership experience.
- Strong clinical assessment skills and knowledge of home care practices; experience in geriatrics is preferred.
- Excellent leadership, communication, and interpersonal skills to promote teamwork and rapport with patients, families, and staff.
- Ability to work independently, manage multiple responsibilities, and adapt to evolving needs.
- Proficient in electronic health records (EHR) and other relevant healthcare technology.
- CPR and First Aid certification required; CPR Instructor certification is a plus.
- Reliable transportation and willingness to travel in the Jersey City, NJ area.
