Hospice Care Center is seeking a Full Time Hospice RN Case Manager (RNCM) to join our dedicated nursing team providing exceptional end-of-life care throughout the community.
Schedule: 7 am - 7 pm, 3 x 12's, Night Shift, Weekends Required
In this rewarding role, you’ll deliver hands-on hospice nursing care, coordinate services, and guide patients and families through every step of their hospice journey. - Hold current unencumbered license as a Registered Nurse.
- Minimum one (1) year experience as an RN in a medical surgical/acute care setting. Hospice exp a plus!
- Must be computer proficient in typing and various programs, including background in EMR.
- Possess and maintains current CPR certification if required by state.
- Current driver's license, vehicle insurance, and access to dependable transportation.
