General Statement of Duties: The Registered Nurse (RN) for Access Team determines initial nursing care needs of hospice patients and enrolls patients in the hospice program. Provides nursing care to patients and families based on assessed needs reflected in the plan of care.
Level of Responsibility: Reports to Nurse Manager, Access Team
Status: Non- Exempt; Full-Time; Hourly
Pay Range: $37.25/hour - $46.00/hour
Schedule: Monday - Friday, 10:00am-6:30pm, with two rotating weekend days per month.
Duties and Responsibilities:
1. *Enrolls patients in the hospice program, as needed. Explains the benefits and challenges of
the palliative approach to patient care. Explains how the hospice care program aligns with their goals and can address their problems.
2. *Assesses physical and safety needs of patients and families as they relate to nursing problems for establishing the plan of care.
3. *Consults with hospice Medical Director or designee regarding hospice eligibility and medical appropriateness of patient and covered medications for admission for hospice services. Documents consultation with supporting medical data in patients’ electronic record.
4. *Establishes and maintains good working relationships with the patient and family, the attending physician, coworkers, and outside agencies.
5. *Provides nursing interventions for problems associated with pain, nausea/vomiting,
constipation etc. and patient/family teaching, according to guidelines reflecting the plan of care. Documents initial medication profile.
6. *Educates patients/family regarding the progression of disease, pain, and symptoms
management, safety, physical components of care, stages of dying, and hospice philosophy.
7. *Initiates the nursing components of the care plan with the consent of the patient and family.
8. Assists patient in the completion of Advance Directives as needed.
9. *Documents nursing assessment and all other patient-related information.
10. *In consultation with Access Nurse Manager assists with phone triage, assessment of patient needs, and patient visits, as needed.
11. Communicates with appropriate care team to provide continuity of care.
12. Provides updates to physicians and pharmacies.
13. *Serves as a resource to physicians on pain and symptom management.
14. Reports abuse and neglect to the appropriate authority following organization policy.
15. *Participates in providing 7 days a week on call assessment coverage to patients/families. Available by phone for the duration of shift.
16. Assists with staffing at the Inpatient Facility if needed.
17. Participates in nursing staff meetings, in-services, and staff training to develop and maintain hospice expertise.
18. *Demonstrates competency as a hospice nurse.
19. *Meets visit average standards established by the organization.
20. Complies with the employee standards.
21. Other duties as assigned.

Education & Experience:
1. Current West Virginia eNLC Registered Nurse license is required. New RN graduates - please specify when you're taking the NCLEX exam.
2. Minimum of one year nursing experience, home health or medical surgical nursing in an inpatient setting preferred.
3. Ability to establish and maintain good working relationships with the patient and family, the attending physician, coworkers, and outside agencies.
4. Should have demonstrated ability at resolving customer issues and communicating effectively with customers.
5. Able to provide your own transportation, and a license to drive in West Virginia with current liability insurance. Driving record meets Hospice’s insurance company standards.
6. Excellent computer word processing skills.
Physical Qualifications:
1. Ability to transport self to the office and patient care settings throughout the service area.
2. Ability to walk from car to patient’s room in their care setting and perform essential job functions.
3. Ability to lift and move patients and equipment.
4. Ability to communicate verbally and in writing in English language.
5. Ability to hear normal conversation in person and via phone.
6. Ability to read normal or condensed print.
