Overview:
The Housing Works Emergency & Transitional Housing Program (ETH) provides time limited transitional housing and support services to individuals reentering the community following periods of incarceration or other justice system involvement. Program participants engage in a variety of onsite services, including case management, vocational training, housing advocacy and mental health support that address barriers to accessing housing in the community and help build skills that participants can use to achieve and maintain stable functioning in the community. The Housing Case Manager provides direct case management and housing placement assistance services to participants enrolled in the ETH Program, including, but not limited to, conducting psychosocial assessments, co-developing and reviewing housing and treatment plans, connect individuals to community-based services to address social determinants of health, benefits and entitlements enrollment and assisting participants with identifying and preparing for more permanent housing. This is an ideal opportunity for reentry or housing services professionals with case management or housing placement experience looking to work alongside like-minded professionals committed to improving outcomes for justice involved individuals.
Responsibilities:
Primary:
- Manage a caseload of approximately 35-45 clients enrolled in the ETH Program.
- Conduct psychosocial assessments with all newly enrolled clients.
- Conduct housing eligibility screenings with clients to inform and develop the housing plans of care.
- Co-develop robust Treatment Plans and Housing Plans with clients assigned to caseload and review every 30 days.
- Meet with clients regularly in accordance to program workflow and contract deliverables.
- Assist clients with obtaining, or retrieving, vital documents such as a government issued IDs, social security cards, birth certificates, etc.
- Assist clients with benefits and entitlements enrollment.
- Assist clients with housing applications and scheduling apartment viewings.
- Work with clients to assist with family reunification.
- Connect clients to internal and external health, wellness or other support services.
- Escort clients to service appointments as needed and assist with coordinating alternative transportation services.
- Engage in care coordination activities with internal and external providers.
- Conduct case conference meetings with clients interdisciplinary care teams.
- Engage in crisis intervention as needed.
- Maintain accurate and up-to-date client charts utilizing Housing Works electronic medical records systems.
- Adhere to program and agency quality assurance standards.
- Participate in quality improvement activities.
- Submit daily, weekly, monthly and ad hoc reports in a timely manner.
- Participate program and agency meetings.
- Promotes Housing Works’ core values in day to day activities.
- Performs other tasks as assigned by supervisor.
Secondary:
- Provide culturally competent services in accordance to Housing Works policies and practices.
- Engages in advocacy efforts as outlined in Housing Works’ and the Justice Initiatives Department policies.
- Ability to work with clients with diverse clients with varying levels of service needs.
- Works collaboratively with other team members within the Justice Initiatives Department and cross-agency.
Other functions that may be required with this position include:
- Attend and participate in staff meetings and trainings as scheduled.
- Responsiveness to emails, texts and phone calls.
- Flexibility to work pre-scheduled evening hours as needed to meet client service deliverables.
Requirements:
- Bachelor’s Degree required in social services, psychology, social work or other related field.
The following are qualifications and requirements of the position:
- Experience providing case management or direct client service.
- Experience working with individuals involved in the criminal legal system, as well as individuals with substance use or mental health diagnoses.
- Strong written documentation and communication skills
- Good problem-solving skills and ability to work autonomously
- Microsoft Office Suite, particularly Excel, required
- Experience utilizing EMR systems.
The following additional competencies are viewed as important to success in this position:
- Preferably bi-lingual in Spanish but not required.
- Excellent written and oral communication skills.
- Excellent or proficient technical skills to perform tasks related to Microsoft Office, email and case management systems.
- Ability to work independently and collaboratively with the team.
- Dependability and reliability to arrive and depart timely for scheduled shift.
