Job Description:
The Advanced Practice Pharmacist (APP) in the Pharmacotherapy Clinic at John Muir Health plays a vital role in the comprehensive management of patients with a variety of complex and chronic disease states. Practicing under a Collaborative Practice Agreement (CPA) in accordance with California State Board of Pharmacy regulations, the APP is authorized to prescribe, initiate, adjust, and discontinue medications to optimize therapeutic outcomes across a range of specialties. Core responsibilities include assessing medication regimens for safety, efficacy, and adherence; streamlining therapies when appropriate; mitigating drug interactions; and educating patients and caregivers to support long-term treatment success. Pharmacists will perform clinical assessments, order and interpret laboratory tests, and monitor patient responses to therapy to ensure safe, timely, and cost-effective care. The APP collaborates closely with physicians and the care team to ensure continuity of care and achievement of clinical goals. Strong documentation and communication skills are essential in both patient-facing and interdisciplinary team settings. In addition to direct patient care, the APP contributes to policy development, quality improvement, and staff education. Responsibilities may also include supervising technicians, supporting regulatory compliance initiatives, and assuming leadership duties in the absence of pharmacy management. Success in this role requires clinical expertise, adaptability in a high-acuity outpatient environment, and a proactive, collaborative approach to patient care.Education:
Doctor of Pharmacy (PharmD) or Bachelor of Science (BSPharm) from a school accredited by ACPE - Required
Experience:
Candidates must meet qualifications consistent with California State Board of Pharmacy requirements for Advanced Practice Pharmacist (APP) recognition by fulfilling any two (2) of the following:
Current board certification in a relevant clinical area (e.g., BCACP, BCPS, BCGP, BCOP, or other board-approved specialty certification).
Completion of an American Society of Health System Pharmacists (ASHP) accredited PGY-1 or PGY-2 residency in Ambulatory Care, Pharmacotherapy, Cardiology, Internal Medicine, Oncology, Geriatrics, Specialty Pharmacy, or other applicable specialty.
At least 1,500 hours of direct patient care experiences within the past 10 years under a CPA or similar structured clinical protocol.
For candidates without residency training, a minimum of five (5) years of clinical experience in ambulatory care or chronic disease state management is strongly preferred.
Additional Experience Requirements:
Ability to multitask and adapt to a high-acuity outpatient environment.
Demonstrated ability to perform in a complex and collaborative clinical setting.
Must be able to educate, monitor, order, and follow labs for patients, related to metabolic disease states.
Experience in developing policies and engaging in continuous process improvement.
Certifications/Licensures:
RPH Registered Pharmacist - California Board of Pharmacy - Required
BLS Basic Life Support - American Heart Association – Required
APh Advanced Practice Pharmacist License – Required within six months of hire
Board Certification in a pharmacy specialty recognized by the Board of Pharmacy Specialties – Required for positions where it is relevant to the assigned area of practice (e.g., BCOP for oncology, BCPS for pharmacotherapy, BCACP for ambulatory care, etc.)
