SALARY
This position is not eligible for overtime (exempt); however, the city has a compensatory time leave program for employees that grants leave time for hours worked in excess of forty hours.

GENERAL DESCRIPTION
The purpose of this position is to provide advanced, ongoing case management and social services to Fayette County¿s residents age sixty and older to improve life and increase self-sufficiency and stability. This position is located at the Lexington Senior Center.
MINIMUM REQUIREMENTS
- Bachelor's degree in social work
- Two (2) years of related experience
- Additional years of related education may substitute for years of experience
- Valid Social Work License from the Kentucky Board of Social Work
ESSENTIAL FUNCTIONS
- Provides advanced case management, crisis intervention, and social services to assist with educational, recreational, financial, insurance and benefits counseling, enrollment, support, and problem resolution
- Coordinates and distributes food assistance through Farmers Market programs and USDA Commodity distributions
- Completes and processes applications for Financial Assistance, WHEELS transportation services, SNAP, and other benefit programs
- Conducts initial and ongoing clients' assessments to create, monitor, review and update individualized service plans

Provides and coordinates the provision of required services for the client and makes referrals as needed
- Assists individuals and families with accessing community resources and public assistance programs
- Provides information, referrals, and follow-up services to ensure clients receive appropriate assistance

Enters and maintains accurate and up to date case notes and data; provides needed documentation, writes reports, and performs other administrative tasks as needed
- Keeps records and documentation in compliance with agency policies and program guidelines
- Administers and compiles surveys to evaluate programs, gather community feedback, and support reporting requirements

Creates, contacts, and maintains relationships with community partners, agencies, and service providers
- Collaborates with staff, community partners, and stakeholders to support agency programs and services
- Performs outreach and customer service duties to further the mission of the organization

Coordinates and/or attends treatment team meetings, court hearings, and other various internal and external meetings, as needed
- Represents the organization at community events, outreach activities, health fairs, and public presentations
- Transports client(s) to essential meetings and appointments, as needed
- Provides supervision of social work or multidisciplinary student interns, as assigned
- Performs related work as assigned
SPECIAL LICENSE/CERTIFICATION
All Certifications and licenses must be maintained as a condition of employment.
- Valid driver's license
- Valid Social Work License from the Kentucky Board of Social Work
- First Aid and CPR/AED Certification must be obtained within first six (6) months of employment
SPECIAL REQUIREMENTS
- May be required to work evening hours and/or weekends as assigned
- All positions require drug testing, criminal records check, Central Abuse Registry check, Nurse Aide Abuse Registry check and APS Caregiver Misconduct Registry check before employment and may require a post-job offer physical as stated in Ordinances 21-14(b), 22-13 and 23-16.
