LSSI is hiring a full-time CST Case Manager, Monday through Friday. Responsible for providing whole-person services to clients participating in mental health case management programs, including 1:1 meetings, case management assistance, and the utilization of evidence informed interventions that target behavioral, emotional, and social skill building. Also responsible for assisting clients in achieving their stated goals and objectives; collaborating with internal and external treatment professionals and community partners.

- Collaborate with clients and/or colleagues to complete lM+CANS, treatment plans, and other required documentation.

- Understand and reflect the clinical model of the program, demonstrating principles of Equity, Diversity, and Inclusion.
- Adhere to all timelines regarding documentation completion.
- Ensure adherence to ethical standards of treatment, including NASW Code of Ethics.
- Demonstrate professional, positive behavior and carry out responsibilities with integrity, treating clients, families, other LSSI workforce members, and collaborative organizations and/or individual in a dignified, respectful, honest, and fair manner.
- High school diploma or equivalent required.
- Associate’s, bachelor’s, or master’s degree in a social service field preferred.
- If master’s level, work toward clinical licensure.
- LPC/LCPC or LSW/LCSW preferred.
- Background check clearance required.
- Demonstrated proficiency in Electronic Health Record documentation and review required.
- Clinically trained and experience using best practices as well as experience working with a variety of client ages, issues, and symptoms preferred.
- Familiarity with accreditation and licensure standards preferred.
- Experience establishing and maintaining collaborative community partnerships preferred.
- Demonstrated ability to communicate in a clear, comprehensible manner, both verbally and in writing.
- Excellent organization, presentation, and pc/computer skills, including experience using Microsoft Office (Outlook, Teams, Word, Excel, PowerPoint) along with other related software.
- Must Have:
- Valid driver’s license, in good standing for the state of residency required.
- Access to reliable transportation required.
- Valid IL statutory minimum liability insurance coverage, bodily injury and property damage required.
