JOB SUMMARY: Works as a liaison between Therapy, Nursing, Medical Staff and other members of the Interdisciplinary Team to promote the goals and objectives of the organization and maintain standards of nursing/clinical services in the rehabilitation setting.
PRINCIPLE ACCOUNTABILITIES
- Demonstrates strong clinical, leadership, and management skills; good interpersonal and interdisciplinary relationships; a high level of initiative and creative problem-solving ability, so that an environment conducive to providing excellence in rehabilitative nursing care and staff development can be maintained.
- Assists with the development, implementation and evaluation of the standards of patient care within the rehabilitation setting, ensuring compliance with all operating policies and practices, as well as regulatory requirements.
- Makes suggestions for/monitors the capital and operational budget for the Capital Neighborhood; regularly reviews expenses and revenue and justifies variances, as requested.
- Performs other related duties as assigned or directed.
ESSENTIAL JOB FUNCTIONS
Rehabilitation Care and Services
- Implements strategies and programs for rehabilitation care consistent with the mission, vision, policies, goals and objectives of the Masonic Care Community.
- Assists with the development/implementation of strategic planning for the rehabilitation setting that focuses on the provision of high-quality, cost-effective care for the resident requiring rehabilitation services.
- Evaluates the quality and appropriateness of care and effectiveness and efficiency of systems within the rehabilitation setting; recommends/implements practice changes based on evaluation and supports interdisciplinary performance improvement efforts.
- Ensures that current rehabilitation research findings and rehabilitation evidence-based practices are incorporated into clinical practice and delivery of care.
- Maintains knowledge of trends in rehabilitation and healthcare and works collaboratively with the Therapy and Medical Staff to identify the impact of the trends on the delivery of rehabilitation care.
- Promotes professional rehabilitation nursing and rehabilitation values, goals and objectives among nursing staff and interdisciplinary team members (i.e. resident’s achievement of independence, maintenance of optimal health and quality of life, avoidance of complications leading to hospital re-admission).

- Encourages rehabilitation nursing staff to use the nursing process, critical thinking, and ethical practice in managing care and provides for continuity from admission through discharge and successful reintegration into family, community, and/or alternate discharge setting.
- Collaborates with medical staff, interdisciplinary team members, residents and families in coordinating an inter-disciplinary approach to resident’s plan of care.
- Supports the rehabilitation nursing staff in performing the clinical roles of educator, caregiver, and resident advocate.
- Provides an environment which enables the rehabilitation staff to participate actively in setting goals, defining programs, and evaluating outcomes.
- Participates in interdisciplinary committees and other activities within the organization.
Human Resource Management
- Maintains appropriate staffing levels and skill mix among staff members.
- Accountable for the management of the Neighborhood Staff assigned to the Rehab Households, in compliance with Human Resource policies.
- Assures that performance evaluations on assigned staff are completed at least annually.
- Promotes an atmosphere conducive for professional growth and development by acting as a role model, mentor and coach to the Rehab household staff; providing developmental opportunities for staff to acquire clinical rehabilitation skills and expertise.
- Identifies, analyzes and plans for staffing needs, taking into account resident acuity and staff competencies to provide efficient, safe, and effective care of rehabilitation residents.
Professional Growth and Development
- Seeks continuing education necessary for advancing professional rehabilitation management skills.
- Is an active member of the Association of Rehabilitation Nurses and serves as an advocate for rehabilitation nursing.
- Regularly attends departmental meetings as established by the Masonic Care Community.
JOB QUALIFICATIONS:
- NYS licensed RN with CPR certification.
- BS in Nursing preferred.
- Certification in Rehabilitation Nursing preferred.
- Previous Long Term Care experience required.
- Supervisory experience preferred.
- Must possess basic knowledge of electronic health records.
- Must demonstrate strong clinical, leadership, and management skills; good interpersonal and interdisciplinary relationships; a high level of initiative and creative problem-solving ability.
Job Type: Full-time
Pay: $95,000.00 - $105,000.00 per year
