Schedule: Monday - Friday, Afterhours, 4:00 PM - 12:00 AM
Weekend Rotation: Every 8th weekend.

At Mountain Valley Hospice, we offer compassionate, personalized care to patients and their families during life’s most challenging moments. We are currently seeking a dedicated and skilled Licensed Practical Nurse (LPN) to join our team of professionals, providing exceptional care in patients’ homes, skilled nursing facilities (SNFs), and assisted living facilities (ALFs). As a part of our team, you’ll collaborate with nurses, social workers, chaplains, and providers who share a deep commitment to delivering high-quality care and support. Join us in making a meaningful difference in the lives of those we serve.
What You'll Do:
- Deliver Exceptional Care: Provide direct care to patients, including administration of medication, wound care, treatments, and procedures under the supervision of a registered nurse.
- Collaborate: Work with the interdisciplinary team to address patient and family needs, including connecting with other providers (aides, social workers, chaplains, and more).
- Monitor and Respond: Recognize and document changes in patient conditions and communicate these effectively to the care team.
Your Schedule:
- Monday - Friday, Afterhours, 4:00pm-12:00am.
- Weekend Rotation: Every 8th weekend.
Qualifications
- Valid Licensed Practical Nurse License
- Exceptional interpersonal and communication skills, with a compassionate and patient-centered approach.
- Ability to work both independently and as part of a team, with excellent time management skills.
