Job Overview
Join our tech-enabled medical group in Everett, Washington as a Nurse Practitioner or Physician Assistant, where you will play a pivotal role in delivering hospital-level acute care directly into patients' homes across Snohomish County. This is a field-based, high-acuity role with a 3 x 12-hour shift schedule — four consistent days off every week — backed by a dedicated team of paramedics, nurses, and physicians, and a mission that is measurably reducing hospitalizations and improving lives across the community.
You'll take your high-acuity clinical skills out of the facility and into the home — assessing, deciding, and acting in real time, with the autonomy to own the care plan and the team support to do it exceptionally well.
Responsibilities
- Perform problem-focused assessments and hands-on interventions in the home including IV fluids, wound care, splinting, and medication administration
- Order and interpret diagnostics including labs, EKGs, and point-of-care ultrasound (POCUS)
- Partner with referring clinicians to build holistic care plans that honor patient goals and palliative principles
- Build meaningful, ongoing relationships with patients and families across their health journey
- Coordinate care transitions and collaborate with the interdisciplinary team to support safe handoffs and continuity
- Contribute to quality improvement initiatives focused on reducing readmissions and ED utilization
- Maintain accurate, timely clinical documentation in compliance with regulatory requirements
- Transport and deploy field equipment across diverse home settings throughout Snohomish County
Requirements
- Active, unrestricted NP or PA license in Washington State required
- FNP, AGACNP, or AGPCNP certification accepted for NPs
- Minimum 2 years (or 3,600 hours) of high-acuity experience required — ED, inpatient medicine, or post-acute care
- Current BLS required — ACLS preferred
- Valid driver's license required
- Ability to carry equipment up to 60 lbs and navigate stairs required
- EMS background with IV access and field medication administration strongly preferred
- Comfortable operating in fast-paced, startup-style environments
- Experience with home-based or hospital-at-home care models preferred
