The Opportunity
As a Part-Time Home Health Occupational Therapist, you will help patients regain the skills and confidence needed to live safely and independently in their homes.
You will work closely with patients, families, nurses, physical therapists, physicians, and other members of the interdisciplinary team to address functional limitations following hospitalization, illness, injury, or a decline in condition.
This position is ideal for an OT who enjoys developing meaningful relationships with patients and providing individualized care in a one-on-one environment.
Responsibilities
Complete comprehensive occupational therapy evaluations and reassessments
Develop individualized plans of care based on each patient’s functional needs and personal goals
Provide skilled treatment focused on activities of daily living, upper-extremity function, strength, coordination, cognition, and safety
Assess home environments and recommend modifications, adaptive equipment, and assistive devices
Provide training related to bathing, dressing, toileting, meal preparation, and other daily activities
Educate patients, families, and caregivers on safety strategies and therapeutic exercises
Coordinate care with nursing, physical therapy, physicians, and other providers
Participate in interdisciplinary communication and care planning
Complete accurate and timely clinical documentation
Communicate changes in condition and safety concerns promptly
Qualifications
Current Connecticut Occupational Therapist license
Graduate of an accredited occupational therapy program
Valid driver’s license and reliable transportation
Strong communication, organization, and documentation skills
Ability to work independently while collaborating with the clinical team
A positive attitude and commitment to patient-centered care
Home health experience is preferred, but we welcome occupational therapists with strong clinical skills who are motivated to learn and grow in the home health setting.
