Job Type: Part Time and Hourly, with the possibility for Full Time
Job Location: Client located in Sanford, Colorado
Compensation: $18 - $22 dependent upon experience
Benefits for Full-Time Employment: Health, Vision and Dental Insurance, HSA, PTO/Sick Time, 403b with Company Match, Voluntary Life and AD&D, Short/Long Term Disability.
Essential Functions:
- Provides direct care to selected clients as assigned by nursing leadership.
- Provides skilled assistance with activities of daily living, i.e., bathing, combing hair, oral hygiene, dressing, etc.
- Provides care in accordance with the client’s plan of care and tasks assigned by RN.
- Implements the client's plan of care in compliance with agency and governmental regulations.
- Provides simple exercises, transfers, and assists with therapy programs when instructed.
- Responsible for documentation of all care provided within 24 hours of the visit utilizing electronic health record software.
- Reports immediately to supervisor or professional staff any changes in the client's condition or any problems in assigned tasks.
- Provides emotional support to clients and families.

- Ability to effectively communicate with staff, clients and medical personnel.
- Ability to advocate for and relate to clients/families and their concerns.
Minimum Experience: 1-year experience working as a CNA preferred. Home care experience preferred.
Licenses/Certifications: Must have a Colorado CNA certificate in good standing (DORA). Current BLS/CPR required.

The employee is occasionally required to stand; walk; sit; and reach with hands and arms. The employee is regularly required to drive to client homes.
Working Conditions:
Home environment. May be exposed to occupational hazards such as communicable diseases, harmful chemicals, and/or disoriented or combative patients. Must be able to work under stress and be emotionally capable of coping with changes in activity.
