Help Patients and Families Find Support When They Need It Most
Ohio's Hospice is looking for a compassionate Medical Social Worker to provide care and support to patients receiving hospice services throughout Newark and the surrounding communities. In this role, you'll work directly with patients and caregivers to address emotional, social, financial, and practical needs while partnering with an experienced interdisciplinary team focused on comfort and quality of life.
This is an excellent opportunity for a licensed social worker who enjoys working independently in the field while building meaningful relationships with patients and families.
Work Schedule
- Monday through Friday
- 8:30 a.m. – 5:00 p.m.
- Shared weekend and holiday rotation
- Regular travel to patient homes, nursing facilities, and assisted living communities within the service area
Your Role
Every day brings an opportunity to make a lasting impact. As a member of the hospice care team, you will:
- Evaluate the psychosocial needs of patients and their families.
- Develop individualized plans that support each patient's goals and preferences.
- Provide counseling, education, and emotional support throughout the hospice journey.
- Connect families with financial assistance, community agencies, and other available resources.
- Assist with advance directives and end-of-life planning conversations.
- Support caregivers experiencing stress, anticipatory grief, or bereavement.
- Partner closely with nurses, physicians, chaplains, hospice aides, and volunteers to coordinate patient-centered care.
- Complete accurate and timely documentation in the electronic medical record.
- Advocate for patient choice while promoting dignity, comfort, and quality of life.
Qualifications
Required
- Bachelor's degree in Social Work.
- Current Ohio LSW or LISW license.
- At least one year of experience in a healthcare environment.
- Valid driver's license with reliable transportation and required automobile insurance.
- Strong communication, organization, and documentation skills.
- Ability to travel throughout the assigned service area in various weather conditions.
Preferred
- Master's degree in Social Work (MSW).
- Previous hospice, home health, oncology, geriatric, or palliative care experience.
- CHP-SW or ACHP-SW certification.
- Current CPR certification.
- Notary Public designation or ability to obtain within six months of employment.
