Job Summary
Applies advanced knowledge/clinical skills necessary to assess/manage the healthcare of chronically ill geriatric participants in the PACE of Guilford and Rockingham Counties Program.
Specifications
Education: Bachelor’s degree in nursing from accredited college/university; graduate of certified or accredited Nurse Practitioner program.
Experience: 3-5 years nursing experience with 1-3 years’ experience as a Nurse Practitioner.
One year experience with the frail or elderly.
Number and Type of Employees Supervised (optional): None
Licensure, Registry or Certification Required: RN licensure in NC and certification from Nurse Practitioner program.
Special Training: Ability to learn, teach and research; ability to solve problems and think analytically; must possess skills necessary to treat geriatric patients, ability to get along with others in a team environment. Only act within the scope of his or her authority to practice. Meet a standardized set of competencies for the specific position description established by PACE of Guilford and Rockingham Counties and approved by CMS before working independently.
Immunizations: Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact

Key Responsibilities (*denotes an age-related skill or task)
- Performs health histories, physical exams and patient teaching. Orders, interprets and follows up on lab tests and results. Performs selected procedures and documents care orally and in writing. Initiates emergency measures as indicated. Recognizes abnormalities that justify immediate referral for intervention by others.
- Initiates, evaluates, reviews and changes orders for selected medications and diagnostic studies/treatments within attending physician approved protocols.
- Assesses, initiates and coordinates patient care on a daily basis under the supervision of the attending physician.
- Requests consultations from all services as needed in collaboration with attending physician and/or established protocols.
- Acts as liaison with physicians, nurses, and other healthcare professionals within the program, providing education, communication and support for healthcare staff, patients and families. Directs patient management in conjunction with physician staff.
- Participates as a member of the Interdisciplinary Team. Attends staff meetings and takes part in participant care planning. Communicates any changes in participant condition to team members.
- This position requires support by working occasional on-call Monday-Sunday.
