Overview
We are seeking a full-time Nurse Practitioner to provide high-quality care to patients in skilled nursing facilities throughout the Hollywood area. This position involves rounding twice per week at assigned facilities, seeing approximately 25 patients per day. The ideal candidate is compassionate, clinically confident, and experienced in caring for the geriatric population.
Responsibilities
- Provide comprehensive medical care to residents in skilled nursing facilities.
- Conduct physical assessments, evaluate acute and chronic medical conditions, and develop treatment plans.
- Manage approximately 25 patients per day during scheduled facility rounds.
- Round at assigned Hollywood-area nursing homes twice weekly as part of a full-time schedule.
- Prescribe medications, order diagnostic tests, and interpret results.
- Coordinate care with physicians, nursing staff, therapists, and interdisciplinary teams.
- Document patient encounters accurately using the electronic medical record (EMR).
- Participate in care planning meetings and ensure continuity of care.
- Educate patients, families, and facility staff on treatment plans and disease management.
- Maintain compliance with all state, federal, HIPAA, and facility regulations.
Qualifications
- Current California Nurse Practitioner license in good standing.
- DEA license and furnishing number (or ability to obtain).
- Experience in skilled nursing, long-term care, geriatrics, internal medicine, family medicine, or hospital medicine preferred.
- Strong clinical assessment and decision-making skills.
- Excellent communication and interpersonal abilities.
- Proficiency with electronic medical records.
- Ability to work independently while collaborating effectively with healthcare teams.
Schedule
