Partners In Home Care (PIHC) has been delivering compassionate, high-quality home-based care across Western Montana since 1987. Our Quality Assurance team brings deep knowledge of home health, hospice, and assistive care to support patients, providers, and our clinical teams. We are mission-driven and patient-first in everything we do: from the way we answer the phone to the way we review a Face-to-Face note.
We're looking for an LPN who thrives on relationship-building, loves the detail work of clinical documentation, and wants to make a difference in the Missoula Community. This is a newly created position — built intentionally to deepen our support for community providers, strengthen the patient experience, and stay ahead of evolving regulatory standards.
The Role
As our Clinical Intake Specialist, you are a valued clinical presence in the office — the LPN who brings a layer of clinical expertise to our Intake team's daily work and a trusted, familiar face to our referral sources. You'll assist in review of incoming Home Health and Hospice referrals for clinical appropriateness, lead Face-to-Face (F2F) documentation review, and serve as the go-to clinical resource for Intake staff navigating complex cases and compliance concerns. You're part of our Quality team, which means your work directly shapes the standards that keep patients safe and ensure our clinicians are fully prepared at every admission.
What You'll Do
Build & Maintain Referral Source Relationships
- Serve as a representative of PIHC with hospitals, clinics, physician offices, and community partners across the Missoula region — visiting referral sources in person to strengthen partnerships and educate on our services alongside our Intake team.
- Communicate proactively about admission timelines, documentation needs, and service capabilities — keeping referral sources informed and confident in choosing PIHC.
- Identify gaps in referral source knowledge and deliver education on Home Health and Hospice eligibility, documentation requirements, and patient pathways, alongside our Intake team.
- Represent PIHC's mission and quality standards in every interaction.
Face-to-Face Documentation Review (LPN Clinical Function)
- Review F2F encounter notes for Medicare and payer compliance, confirming: provider signature and date, required timeframes, homebound status (Home Health) or terminal prognosis (Hospice), and clear medical necessity documentation.
- Identify incomplete or non-compliant documentation and coordinate directly with providers to resolve — reducing billing risk and admission delays.
- Monitor outstanding F2Fs and escalate proactively to prevent gaps in care.
- Apply clinical judgment to assess referral appropriateness before admission.
Clinical Support for the Intake Team
- Work shoulder-to-shoulder with the Intake team— answering questions, guiding eligibility decisions, and helping triage complex or uncertain referrals.
- Receive, process, and track incoming Home Health and Hospice referrals from receipt through admission.
- Verify patient demographics, insurance coverage, and payer criteria; ensure referral documentation is complete before admission proceeds.
- Communicate admission readiness to clinical managers and coordinate start-of-care timing with scheduling and field clinicians.
- Enter and maintain accurate referral and patient data in the EMR (Home Care Home Base preferred; experience with Epic, Cerner, or similar EMRs considered).
Quality Team Contribution
- As a member of the Quality team, uphold the metrics and standards that ensure every patient admitted to services is fully supported from day one.
- Serve as the clinical bridge between the Quality and Intake teams — bringing a clinical lens to intake workflows and surfacing patterns that inform quality improvement.
- Participate in audits, quality reviews, and process improvement initiatives.
- Identify trends in documentation deficiencies and share findings with the Quality team to drive systemic improvement.
- Support compliance with Medicare/Medicaid, accreditation standards, and HIPAA requirements.
- Other duties as assigned.
What You Bring
Required
- Active Licensed Practical Nurse (LPN) license in Montana (or ability to obtain prior to start date)
- Minimum 2 years of clinical experience — Home Health, Hospice, hospital, SNF, or case management preferred
- Knowledge of Medicare Home Health and Hospice eligibility criteria
- Strong clinical documentation skills and understanding of medical terminology
- Excellent interpersonal and customer service skills — you genuinely enjoy building relationships with referral sources, physicians, and community partners
- Comfort with in-person outreach and relationship development (this role includes face-to-face visits with referral sources)
Preferred
- Prior intake or authorization experience in Home Health or Hospice
- Experience reviewing Face-to-Face encounter documentation for Medicare compliance
- Familiarity with Home Care Home Base (HCHB) EMR; Epic or Cerner experience also valued
- Knowledge of PDGM (Home Health) and Hospice Conditions of Participation
- ICD-10 Coding knowledge and/or certification
- Established relationships with Missoula-area referral sources or healthcare community
