Summary:
Promotes, maintains, and improves individual and community health by assisting individuals and communities to adopt healthy behaviors, through management of the RHTP - Community Health Worker (CHW) Program, planning/coordination of disease management and prevention projects, and delivery of (CHW) services.
Essential Responsibilities and Duties:
- Supervise RHTP CHW’s under clinical direction of the Population/Public Health Director.
- Submit monthly program reports to Population/Public Health Director regarding agreed metrics with the state of NE RHTP that are required for the program.
- Assist in development of plans and policies necessary to achieve community health goals/objectives that have been outlined in the NE RHTP that are required for the program.
- Develop and coordinate disease management and prevention programs across sites.
- Evaluate CHW program progress and address needs for improvement.
- Deliver CHW services, population health and disease management, health education and promotion programs, home visits, proactive care for disease management, chronic care management and case management.
- Facilitate the development of a referral network to assist Tribal members in meeting their health care needs and assess community health care resources to determine and address gaps in care.
- Produce reports as required by NE RHTP funding sources.
- Other duties as assigned.
Relationships and Authority:
This position supervises RHTP Community Health Workers.
Working Environment and Physical Demands:
Physical requirements include carrying/lifting up to 5 pounds, frequently; up to 10 pounds, regularly; and up to 25 pounds, occasionally. Visual acuity, speech and hearing; hand and eye coordination and manual dexterity necessary to drive an automobile and operate computer keyboard and basic office equipment; subject to carrying, lifting, twisting and reaching to perform essential job functions. Working conditions are primarily in an office environment with occasional travel to other locations. At times staff may be required to work with patients/clients who are in crisis and demonstrate empathy, understanding, respect, and advocacy while remaining calm in stressful situations
Qualifications:
- Associate’s degree in public health, nursing, or related field or equivalent combination of experience and education in health, nursing or related field.
- Certified Nurse’s Aide, Certified Medical Assistant, LPN or an equal certification/license.
- Two years of health-related setting experience.
- One year of grant-funded public health program oversight.
- Computer proficiency with Microsoft Word, Excel, and PowerPoint programs.
- Excellent written and oral communication skills.
- Valid driver’s license and be insurable.
- Successfully pass drug and alcohol testing, motor vehicle, education, character, and criminal background checks.
- Able and willing to travel.
Preferred:
- Bachelor’s degree in health-related field, nursing, or public health.
- LPN or RN
- One year of experience in grant and budget management.
- One year of experience working with Native Americans or Native American organizations.
- Knowledge and understanding of Public Law 93-638: Indian Self-Determination Act.
Preference will be given to Ponca Tribe of Nebraska members or members of a federally recognized tribe. However, all qualified individuals are eligible.
The Ponca Tribe of Nebraska is an at-will employer.
This position is funded through a CMS and State-funded initiative. Continued employment is contingent upon the availability of funding.
