Nurse Practitioner (Full-Time)

This role is ideal for a motivated clinician who values autonomy, collaboration, and continuous professional growth. You'll work alongside a dedicated healthcare team while expanding your clinical expertise in vascular care management.
Local travel is an essential part of this role, with approximately 75% travel required to support patient care needs and collaborate with healthcare partners throughout the community.
What You'll Do
- Perform comprehensive patient assessments, physical examinations, and medical history reviews.
- Order, interpret, and evaluate diagnostic testing, including laboratory studies and imaging.
- Diagnose and treat acute and chronic vascular conditions.
- Develop, implement, and manage individualized treatment plans.
- Prescribe medications and monitor treatment effectiveness.
- Educate patients and caregivers on health conditions, preventive care, and treatment plans.
- Collaborate with referring providers and interdisciplinary care teams to ensure seamless patient care.
- Maintain accurate and timely clinical documentation.
- Monitor patient progress and adjust care plans as needed to achieve optimal health outcomes.
What We're Looking For
- Experience practicing as an Advanced Practice Nurse in clinical settings such as primary care, hospitals, outpatient clinics, emergency medicine, or related healthcare environments. New APN graduates considered.
- Strong clinical assessment and diagnostic skills.
- Knowledge of healthcare regulations, quality standards, and patient care best practices.
- Exceptional communication and patient education abilities.
- Ability to work independently while thriving in a collaborative team environment.
- Excellent critical thinking, problem-solving, and decision-making skills.
- Commitment to ongoing professional development and lifelong learning.
- Bilingual English/Spanish skills are highly preferred.
Required Qualifications
- Current and unrestricted Nurse Practitioner license.
- Active DEA and CDS licenses.
- Current BLS and ACLS certifications.
- Valid driver's license and reliable transportation.
- Willingness and ability to travel regularly throughout the service area.
